**Job Details:**

This clinical pharmacy specialist collaborates with the *Heart Failure, Ventricular Assist Device (VAD), Heart Transplant and Cardiac Amyloidosis Services* to ensure safe, effective and economical drug therapy, particularly in patients who are prescribed chronic, complex, and costly medications. The pharmacist helps to identify barriers to medication adherence considering patient characteristics such as cultural, religious and personal beliefs, socioeconomic status, healthcare literacy and logistics. The pharmacist works closely with the care team and patients to improve access to prescribed drugs and participates in educating patients about drug therapy. The pharmacist helps to identify potential and actual adverse events resulting from drug therapy and recommends the necessary treatment. The clinical pharmacy specialist serves as a consultant to providers on all matters relating to drug therapy.

Responsibilities Include

* Participating in the care of patients seen in the Heart Failure, VAD, Heart Transplant, and Cardiac Amyloidosis clinics

* Gathering, reconciling and documenting patients medication histories

* Evaluating and documenting patients medication taking behaviors and designing strategies for improving drug therapy adherence when necessary

* Collaborating with physicians and advanced practice providers to ensure patients receive safe, appropriate, and affordable guideline directed medical therapy

* Providing support, medication access, and medication education to heart transplant patients as a member of the multidisciplinary transplant team

* Working with patients, pharmacists, pharmacy technicians, nurses, physicians, social workers and case managers to facilitate access to medications

* Working with acute care pharmacists and pharmacy technicians to ensure safe and effective transitions from inpatient to outpatient care

* Collaborating with providers to manage anticoagulation prophylaxis and therapy for patients followed by the VAD program

* Providing clinical and administrative oversight to Department of Pharmacy programs serving the CardioVascular Center

* Maintaining selected institutional guidelines, protocols, policies, and patient educational materials

* Serving as a preceptor for PGY1 and PGY2 residents and pharmacy students

* This clinical pharmacy specialist is responsible for medication management for patients treated in a primary care or a medical, surgical, or pediatric specialty clinic.

* They collaborate with other healthcare professionals to design, implement, monitor, evaluate and modify pharmacotherapy to ensure safe, effective and economical patient care.

* The pharmacist identifies potential or actual adverse events resulting from drug therapy and recommends or initiates the necessary treatment.

* They identify barriers to medication adherence incorporating patient characteristics such as cultural, religious and personal beliefs, socioeconomic status, healthcare literacy and logistics.

* The pharmacist works closely with the care team and patient to improve access to and / or modify drug therapy to overcome barriers.

* Clinical pharmacy specialists provide leadership by facilitating the development, implementation and maintenance of systems that promote desirable patient outcomes.

* They educate professional staff members, students, interns and residents.

* They initiate and participate in drug therapy research.

**Job Qualifications:**

* Bachelor's degree in Pharmacy or a PharmD degree from an accredited college or university is required.

* Current MA license to practice pharmacy or eligibility for licensure required.

* Successfully completed a general pharmacy practice residency, and a specialty residency. Candidates without formal postgraduate training should have at least three years of practice experience in cardiovascular care or transplant care settings. An understanding of prescription benefit plans is desirable.
