Interpret data to evaluate outcomes of care and compare to other recognized benchmarks or outcomes. Promote a culture of continuous quality improvement while incorporating the use of safety tools to anticipate, intervene and decrease risk to reduce avoidable readmissions to improve care outcomes. OTHER Implement the use of technology to coordinate and laterally integrate patient care within, across care settings and among healthcare providers. Facilitate the lateral integration of healthcare services across the continuum of care with the overall objective of influencing, achieving, and sustaining high quality care. Interprofessional healthcare team facilitator focused on the delivery of patient-centered care and the evaluation of quality and cost-effectiveness across the healthcare continuum.
