ESSENTIAL DUTIES AND RESPONSIBILITIES
Collaborates with Senior Director of Outreach, Vice President of Patient Navigation and applicable leadership in The System, maintenance and continued development of patient transitions to The System and community outreach programsConducts nursing assessments and observations of patients referred for admission and similar nursing visits for current hospitalized Health System patientsConducts high level presentations and facilitates meetings with key referral source representativesRepresents The System in the field in order to facilitate referrals and admissions, streamlines the referral process, and conducts referral visits and patient assessmentsAssists with obtaining referral documentation and physician orders when necessaryCommunicates with applicable system locations regarding patient transitionsCoordinates new and existing relationship building with major and minor referral sourcesAssists The System in constructing strategic initiatives designed to explain and educate providers and the community about what The System providesEstablishes and maintains professional relationships with all referral sources, including but not limited to case managers, discharge planners, social workers, physicians and physician practices, and any other community or third party referral sourcesMaintains thorough knowledge of market/community in which PTD services in regards to potential new referral sources, hospital discharge trends, and any other relevant demographic information relevant to the continued growth of The SystemMaintains awareness of competitors in the market and develops profilesMaintains working knowledge and technical skill for the use of hospital referral portal platformsMeets established goals for referrals, admissions, conversion ratios, and other quality metrics determined by leadershipReviews monthly analytic data. Tracks information to determine market trends, and develops action plans based on data relative to key performance indicatorsMINIMUM QUALIFICATIONSHave at least two years of related experience in one of the following: marketing healthcare products or services, long-term care, inpatient rehabilitation or health care community relationsValid Georgia Driver's License with the ability to regularly travel throughout assigned region and as necessaryExcellent customer service, interpersonal and oral/written communication skillsKnowledge of long-term care admission requirements and Medicare, Medicaid, Private Pay and Third Party reimbursementAbility to multi-task and function with little direction EEO / M/ F/ D/ V/ Drug Free Workplace
