Position Summary:

The Medication Management Specialist (MMS) serves as a part of the healthcare team at Conway Medical Center and collaborates with other team members to provide optimal patient care. The medication management specialist accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures. Key components of job include being responsible for exercising judgment and professional etiquette when interacting with clients/customers. Further responsibilities include assessing and collecting data from customers to resolve prescription status issues.

Qualifications:

Education:
  • High School Diploma or GED required.
  • Certified Pharmacy Technician (CPhT) or Licensed Practical Nurse (LPN) required.

Experience
  • Three years' experience in a healthcare setting required.
  • One year experience with prior authorizations preferred.
  • Prior experience on the Cerner EMR platform preferred.
  • Previous experience in an ambulatory infusion center preferred.

Licensure/Certification/Registration
  • South Carolina (or compact state) licensure as LPN or CPhT (SCLLR) in good standing required.
  • Valid US Driver's License required.

Duties & Responsibilities:
  • Virtual/telephonic interactions with patient's and/or caregivers
  • Interviews patients to collect a current medication list.
  • Reviews patient's medication list for accuracy/adherence/discrepancies, assists with medication affordability and adherence.
  • Appropriately documents within electronic health record and collaborates with supervising pharmacist.
  • Centralized refill: Authorizes prescription refills by adhering to approved refill protocols, appropriately notifies pharmacist/physician, when necessary, calls patients to give status updates, and schedules future visits for patients with provider.
  • Follows applicable policies and procedures related to prescription management.
  • Demonstrates proficiency with equipment and software.
  • Effectively communicates with patients, caregivers, team members, and third parties to ensure optimal medication management. Medication management
  • Compile, review, and sort all insurance requests for medication management reviews on behalf of the pharmacist to assist with closing care gaps.
  • Track and monitor authorization and scheduling requests to ensure appropriate follow-up, and provide support resolving claim reimbursement issues.
