Position Summary

The Health Plan Quality Community Health Worker is responsible for collaboration with members and provider groups to promote quality gap closure through education and reporting. The Health Plan Quality Community Health Worker will use their experience in, and knowledge of, the communities they serve to identify and develop plans to overcome barriers to quality care.

Job Description

Key Outcomes:

* Create a positive experience and relationship with members and providers.
* Proactively engage member to manage their own health and healthcare.
* Provide member education in alignment with quality campaigns.
* Provide member education on community resources and benefits.
* Provide culturally appropriate health education, information, and outreach in community settings.
* Assist members in health navigation and selecting in-network appropriate providers to meet care needs.
* Assist members in accessing community-based organization resources with referrals or warm hand offs.
* Provide annual updates on quality measure technical specifications to provider groups.
* Deliver quality gap opportunity reports to provider groups.
* Coordinates collection, analysis, and reporting of quality project deliverables.
* Participates in on-going education and training to improve skills and role-specific certifications or specialization.
* Maintains growth and development through self-directed learning activities and involvement in ongoing community health worker trainings.
* Participates in group meetings to ensure policies, procedures and workflows are up to date and advises on revisions needed.
* Performs other duties as assigned.

Education/Experience:

* Associate's degree in allied health discipline, may be substituted for 2-3 years of experience working in human services or community health related fields.
* Experience human services-related field.
* Experience with care coordination and/or care management
* Experience related to SDOH and Health Equity
* 2 years residing within the local community

Required License(s) and/or Certification(s):

* Community Health Worker Certification preferred.

Skills/Knowledge/Competencies (Behaviors):

* Demonstrates understanding of and alignment with Martin's Point values.
* Knowledge of culture and values of community and familiarity with the resources available in the community
* Excellent written and verbal communication skills.
* Working knowledge of Microsoft Office applications.
* Excellent organizational skills.
* Experience with motivational interviewing.
* Ability to utilize an electronic medical record.
* Proficiency of medical terminology.
* Understanding of the managed care environment.
* Demonstrated understanding of community services and navigating the healthcare system.
