Seeking a Licensed Practical Nurse in Westchester County

Medical office setting, no nights, weekends, or holidays
Description:
+ Interviews and maintains patient's medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAS, PhQ, Pain, Smoke Status, etc)
+ Prepares equipment and assists physician during treatments and examinations of patients.
+ Complete and update all assigned tasks & logs. (i.e. correctly recording shipments and vaccines received in appropriate areas, signing out meds & treatments in log).
+ Initiates follow-up as necessary (as directed by provider) i.e. encounters, labs, close non-billables, etc.
+ Does health teaching as indicated by physician, diagnosis and/or inquiries by patient. Directs patient to RN as necessary.
+ Complete Prior Authorizations for specialized medication and services.
+ Attend case conferences and document case updates in patient record.
+ Collection of urine drug screens.
+ Follow up on patient adherence and document accordingly in patient chart.
+ Collaborate with community and referral resources
+ Participate in quality improvement measures to ensure chart compliance and best practice.
Qualifications:
Active State of NY LPN License
BLS Certification
Bilingual is a huge plus
EMR Experience
Schedule:
Monday-Friday 9:00am-5:00pm
