Reports to: Nurse Manager of Hospice Inpatient
GENERAL SUMMARY
Works under the supervision of the Nurse Manager of Hospice Inpatient. The LPN's primary job function is to provide nursing care to patients and families along the health-illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable to practice within the scope of practice outlined by the Ohio Board of Nursing under the direction of a Registered Nurse. Performs other duties as assigned.
QUALIFICATIONS
Education:
* High School Diploma or successful completion of an equivalent High School Exam required
* Graduate of an accredited school of practical nursing required
Licensure:
* LPN(OH) required
* IV certified
* BLS within 90 days of hire required
Experience:
* Approximately one year as a LPN with experience working with terminally ill patients preferred.
Interpersonal Skills:
* Ability to develop positive interactions with patients, families, caregivers, physicians and staff in order to effectively care for the patient.
* Ability to communicate effectively required.
* Demonstrates empathy and sensitivity necessary to elicit patient and family comfort.
Essential Technical/Motor Skills:
* Ability to use hands for repetitive simple grasping, fine manipulation and pushing/pulling when writing, using equipment in patient care, starting IVs, drawing blood, and transferring patients.
Essential Physical Requirements:
* Physical demand level is heavy. Occasionally lifts in excess of 100 pounds and frequently carries 50 pounds. Stands or walks for one to four hours per day with regular breaks when assessing and interviewing patients and providing skilled nursing. Sits for one to three hours per day when reviewing diagnostic data and completing required documentation. Ability to use hands for repetitive simple grasping, fine manipulation and pushing and pulling when attending to a patient's needs, operating patient care equipment, and recording responses to medications and treatment. Occasionally bends, squats, climbs, crawls, and reaches above shoulder level when rendering skilled nursing.
Essential Mental Requirements:
* Has considerable contact with the general public and customer when performing all aspects of job. Occasionally works under close supervision and also without the assistance of other personnel. Requires the ability to cope with deadlines, adjust to irregular activity schedules, concentrates and pay close attention to detail when performing patient assessments, completing required documentation and preparing and administering medication.
Essential Sensory Requirements:
* Good vision and the ability to hear and speak clearly are continuously required when dealing with patients, families, physicians, and nursing staff. The ability to communicate clearly is required when instructing and comforting patients and their families and maintaining effective contact with physicians and a variety of hospital personnel.
Exposure to Hazards:
* Works in good physical surroundings. Occasionally exposed to dust, odors, body fluids, toxic substances, unpleasant patient care activities, and infectious diseases. Frequently exposed to noise.
Other:
* An understanding of Hospice philosophy, concepts of palliative care, and a willingness to practice in this manner.
JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS
The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.
1. Assists in completion of the nursing assessment of patients and documents appropriately on the patient record.
2. Implements nursing interventions; performs and documents nursing activities appropriate to patient age, size and developmental needs and with attention to the diverse needs of patients and families.
3. Communicates and collaborates with physicians, RNs, and other team members in developing the Plan of Care.
4. Assists with evaluating the patient's response to the Plan of Care and assists with updating the Plan of Care.
5. Provides, delegates and coordinates patient care.
6. Provides appropriate support at the time of death.
7. Facilitates patient/family teaching as appropriate.
8. Assists with discharge planning process.
9. Participates in hourly rounding.
10. Attends and participates in daily report, team meetings, briefings, inservices, department and staff meetings, and other related activities.
11. Adheres to all policies, procedures and regulations regarding patient care, conduct, fire, safety, security and risk management.
12. Pre-cleans, decontaminates and transports contaminated instruments according to policy
13. Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU's
14. Performs other duties as assigned.
