AuthoraCare Collective is currently seeking a Social Worker for our In-patient Hospice Home facility in Burlington. This is a Full-time position working Friday, Saturday and Sunday 8:30am - 7:00pm. (3-10 hour days / 30 hours). BSW or MSW required.
Job Summary:
Responsible for providing comprehensive assessment of the social, emotional and practical needs of the Hospice patients and their support systems. Provides education, resources, and referrals to address patient/family issues of grief and loss, adjustment, care needs and end of life planning. Provides educational programs to the community. Functions as a member of an interdisciplinary team of care.

Education/Qualifications:
* Bachelor's or Master's degree in social work.
* One year of experience in social work in a mental health or medical setting is required.
* Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
