POSITION SUMMARY:

The Clinical Pharmacist II is responsible for the provision of direct patient care. Direct patient care activities include improving patient outcomes by optimizing pharmacotherapy, ensuring medication safety, providing emergency response, and promoting effective transitions of care. This position will primarily provide coverage for a new clinical, ICU-focused, decentralized overnight shift (all adult ICUs + PICU/NICU) but will be cross-trained to provide coverage in the emergency department during the overnight hours. This position has rotating shifts, which include weekends and holidays.

Department: Inpatient Pharmacy

Schedule: Part Time, Per Diem

ESSENTIAL RESPONSIBILITIES / DUTIES:

Direct patient care

* Fills decentralized ICU and/or emergency medicine shifts to provide patient-centered care for critically ill patients during the overnight hours (21:00-07:30)
* Supports the multidisciplinary teams through participation in all aspects of the medication-use system and communication of recommendations to optimize pharmacotherapy
* Participates in the medication management for advanced therapies including, but not limited to, adult ECMO, Impella, and CRRT.
* Serves as the primary pharmacy response for emergency situations (code blue, code stroke, trauma response)
* Provides effective transition of care needs as necessary, including, but not limited to, medication reconciliation, discharge counseling, and facilitating medication to bedside.
* Reports medication-related events to promote the safe use of medications
* Documents all patient care activities
* Follows departmental policies and procedures to promote cost-effective medication use
* Serves as a pharmacy liaison to the BMC staff by ensuring the connectivity between clinical and distributive services.

Distributive and operational responsibilities

* Ensures all medications are handled in the manner established by the Formulary Management Committee, applicable accreditation standards, state and federal regulations, and the Massachusetts Board of Pharmacy.
* Reviews and verifies medication orders, reviews patient charts, evaluates applicable laboratory data to identify or prevent drug-related problems or toxicity.
* Works closely with the central pharmacists and technicians to ensure accurate and timely acquisition, distribution and control of all medications, including investigational drugs. Collaborates with the Pharmacy Purchasing team on special medication needs.
* Adheres to all protocols and policies related to controlled substances.
* Supervises the activities of supportive personnel including student interns and technicians.
* Uses designated pharmacy automation and/or barcoding systems to document checking of any medication or sterile product prior to dispensing.
* Follows departmental policies and procedures to promote cost-effective medication use

Education and training

* Serves as a preceptor for pharmacy students and residents
* Assists in the orientation and training of new pharmacists, interns, and technicians when assigned
* Provides timely and accurate drug information, training, and education to patients and health care providers

Other duties

* Maintains own professional and specialized competency through continuing education as set forth by the Department of Pharmacy and the Massachusetts Board of Pharmacy
* Conforms to Boston Medical Center's standards of conduct so that the best possible customer service and patient care experience is provided
* Conducts interpersonal relationships with patients, visitors, physicians, and fellow employees in a courteous and customer friendly manner.
* Takes corrective action to improve performance on problem areas.
* Maintains all designated hospital information confidential at all times.
* Follows established hospital infection control and safety procedures.

JOB REQUIREMENTS

EDUCATION AND CLINICAL EXPERIENCE:

* Requires a Doctorate of Pharmacy from an accredited College of Pharmacy
* Completion of a PGY1 Pharmacy Practice residency
* Completion of a PGY2 Critical Care or Emergency Medicine residency is preferred (or at least three years of relevant experience)

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

* Work requires a valid license to practice pharmacy in the Commonwealth of Massachusetts. MA pharmacist licensure must be complete and verified by the date of hire.

KNOWLEDGE AND SKILLS:

* Strong clinical knowledge pertinent to the specific practice area
* Strong interpersonal skills when interacting with patients and colleagues
* Involvement in local, regional, or national professional organizations is encouraged
* Proficient with Microsoft Office including Word, Excel and Outlook, and web browsers
* Excellent attention to detail
* Effective verbal and written communication skills; must be proficient in the English; additional language skills are beneficial
