Level of Care Assessor - Registered Nurse
We are currently looking for a Level of Care Assessor Full Time to join our team.
Position Summary:
The Level of Care Assessor is certified by the state to perform a comprehensive assessment that determines if an individual meets a nursing home level of care and qualifies for PACE services.
Qualifications and Requirements:
Education:
Bachelor's degree in a health-related field
RN license in current state
Certification in Choice Assessment (will be trained upon hire)
Experience:
Minimum of 1 year working with a frail or elderly population
Licensure/Certifications:
Valid Driver's License, proof of auto insurance and consistent and reliable usage of a motor vehicle.
Skills and Knowledge:
* Problem solving abilities
* Excellent communication and interpersonal skills
* Basic computer skills
* Knowledge of office equipment (fax/copier)
* Proficient computer skills (word processing, spreadsheets, database)
* Data entry
* Assessment skills
Duties and Responsibilities:
* Evaluates potential new enrollees, determines levels of care, communicates with the appropriate team members. Assessments are conducted in the home of the potential enrollee. Coordinates level of care determinations with the Department of Health and Human Services personnel.
* Successful completion of the annual Choice Training
* Enter evaluation data into the State electronic system
* Effectively communicate to IDT evaluation findings
* Provide comprehensive insight to medical conditions and current medication regimen
* Effectively communicate PACE services
* Completes monthly LOC assessments as directed by supervisor
* Identifies care needs
* Identifies participant's goals and preferences
* Assist in outlining an interim care plan
* Independently schedules appointments with participants
* Documentation is concise, accurate, and meaningful
