Golden Years Homemakers & Companions, a homemaking and companion agency in Plainville, CT is seeking for a Per Diem Licensed Practical and Vocational nurses (LPN) who would like to join our Adult Day Center team. Sessions will be conducted in day center facility. The position is an independent contractor position that will be compensated at a per visit rate.

Responsibilities:

·        Contributes to the assessment of patients by collecting data for analysis.

·        Educates patients/families about care plan for the center, health needs, available resources and follow up care.

·        Oversee clinical quality initiatives; assist with closing care gaps.

·        Maintains safe clean working environment by complying with procedures, rules and regulations.

·        Assists in all duties essential to the operational needs of the site.

·        Adapts positively to a fast-paced challenging environment.

·        Assists individuals to meet their physical, emotional, spiritual, development, health and safety needs.

·        Exhibits effective oral, written and electronic skills.

·        Other Duties as assigned.

 

Qualifications

·        Preferred experience in Adult Day Center/ Rehabilitation Center.

 

 Required Credential(s):

• Licensed Practical/Vocational Nurse credentialed from the Connecticut Department of Public Health obtained prior to hire date or job transfer date.
