  • BASIC FUNCTION

    The Peer Support Specialist provides a unique role in providing supportive services, health, and wellness self-management education by sharing their personal experience of living with HIV, mental illness and/or co-occurring disorders of mental illness, and/or substance use.

    The Peer Support Specialist can extend, provide, and coordinate a broad range of support to individuals who are living with HIV, and/or at risk for HIV by assisting clinical staff with program delivery to individuals who face barriers to accessing care.

    Health Promotion and Care Navigation Initiatives of the Department of Community and Family Medicine are grant-funded programs focused on health disparities. Projects are designed to strengthen an individuals' utilization of primary care and equitable access to prevention, education, and treatment
  • SUPERVISORYACCOUNTABILITY:

    Involves no responsibility or authority for the direction of others.

    NATURE AND SCOPE:

    Internal contacts are with a variety of staff, faculty, and students. External contacts are with staff of the granting agency and program partners.

    PRINCIPAL ACCOUNTABILITIES
    • Conducts screenings to understand the individual's history of high-risk behaviors, drug and/or alcohol use, treatment history, social service and other recovery support needs and motivation to change behavior.
    • Facilitates individual's readiness for change and uses motivational interviewing techniques and other communication skills to support and encourage the patient to plan for harm reduction or elimination of risk behaviors. Models coping techniques and self-help strategies.
    • Assists individuals in linking to community support and treatment services, including helping secure transportation and other resources.
    • Assists individuals in setting personal and linkage to care goals and developing those plans with the Patient Navigator.
    • Works with screening/testing and care team and the individual to connect with community supports and treatment services.
    • Supports families and other loved ones to fully understand their rights, educate them on laws, policies, and procedures, and empower families and other loved ones to effectively articulate their concerns and needs.
    • Acts as a peer support during the inpatient stay, discharge planning and care transitions throughout the continuum of care.
    • Uses technology applications: text, email, video, and patient facing apps to maintain engagement with individuals in various community settings. Electronically reaches out to treatment providers to verify patient showed up for appointment, and follow-up on all missed appointments.
    • Provides health education and health promotion information.
    • Assist with outreach and education activities to increase awareness
    • Participates in training to continually gain new skills.
    • Attends required staff and other meetings.
    • Completes required documentation and other reports.
    • Acts as a power of example in utilizing appropriate and professional conduct.
    • Adheres to IT and business-related communications confidentiality policies.
    • Performs other related duties as assigned.

    CORE COMPETENCIES

    • High motivation to advocate for sexual, ethnic, and racial minorities, immigrants, young people, persons living with HIV, and LGBTQ communities' rights.
    • Active listening skills for providing non-judgmental peer support and engagement to individuals.
    • Must demonstrate a willingness to learn and use digital, computer technology and social media as a form of community outreach and engagement.

    MINIMUM QUALIFICATIONS
    • HS Diploma/GED required, associate degree helpful.
    • Two years minimum experience working in HIV and STI prevention/health promotion field and/or with service delivery to low-wealth individuals, the vast majority of whom are people of color
    • Peer Support Specialist Certification recommended.
    • Knowledge of Washington, DC communities and ability to work with all segments of the community.
    • Ability to communicate effectively both verbally and in writing.
    • Ability to maintain boundaries but comfortable using self and individual experiences as applicable.
    • Proficient computer skills including internet research, email communication and use of a database.
