Overview
The LPN Position in the Department of Medicine provides nursing care for patients and assisting physicians. Clinical and administrative duties including:
(1) triage patients [vital signs, height/weight, medical/social history, medication reconciliation]
(2) stock/prepare exam rooms
(3) retrieve imaging and laboratory results
(4) process work task/message ques in the electronic health records [Allscripts], and the patient portal [Follow My Health]
(5) ability to work in our clinical and administrative areas with the Department of Medicine.
Responsibilities
Triage incoming patients by obtaining vital signs, reconcile current medication list, reconcileallergies, and determine chief complaint.
Administer injections of medications or immunizations in accordance with approved nursingprotocol or standing orders and document appropriately in the patients' electronic medicalrecord. Observe patient for any adverse reactions.
Assist physicians with examinations, minor surgeries, diagnostic procedures, and treatments.Implement standing orders when applicable. Provide basic care as ordered.
Appropriately document all nursing interventions in the electronic medical record.
Review chart for completeness, test results, task completion, and billingProvide patient teaching/education with medications.
Review and work/complete all "tasks" assigned to you and your individual team (as well ascovering for other nurses who may be absent). This will include responding to patientmessages, prescription refills, lab results, referrals, home health agencies, medication priorauthorizations, patient forms and follow up from physicians.
Schedule specialist appointments and obtain referral from insurance companies as needed.Obtain prior authorization for medications.
Obtain lab/CT/MRI/Biopsy results from outside sources, as needed and scan into the patient'selectronic medical record. This includes reviewing upcoming appointments and obtainingprior to the visit.
Notify providers with critical labs
Call test/lab results to patients and answer questions regarding those results and physicianinstructions.
Provides Tuberculin purified protein derivative (PPD) injections as needed.
Respond to questions from patients, family members, faculty/residents, other health careproviders in a timely and professional manner. Patient phone calls should be answered ordocumented the same day they are received. All patient conversation must be documented inthe chart, as well as attempts to reach patient.
Patient education, discharge medication reconciliation, drug assistance programs nutrition andbarriers to care.
Distribute and track Home Health orders and key in Home Health charges for billing.
Participate in staff meeting and training sessions/education lectures.
Other duties as assigned within the office and Department of Medicine.
Qualifications
Completion of a program of instruction for licensure as a Practical Nurse required. Completion of BLS Class and re-certify every 2 years. Must be a licensed LPN in Virginia. Previous clinical experience preferred. Montreal Cognitive Assessment (MoCA) certified desirable.
