Overview
Street Medicine provides direct healthcare to unhoused individuals, wherever they are, with a strong focus on assessing and responding to their physical, social, and psychological identified needs. As an LVN on the Street Medicine team, you will provide essential nursing care, contribute to community health initiatives, and work collaboratively with a multidisciplinary team to promote health and well-being among vulnerable populations.
Enhanced Care Management (ECM) is a statewide Medi-Cal benefit available to select members with complex needs. Enrolled members receive comprehensive care management from a lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, as well as social services. ECM facilitates access to the right care at the right time, in the right setting, beyond traditional healthcare environments.
Community Supports (CS) are services provided by Medi-Cal managed care plans (MCPs) that address health-related social needs, promoting healthier lives and reducing the need for higher, costlier levels of care.
Key Responsibilities
- Patient Care
- Conduct health assessments and monitor vital signs.
- Administer prescribed medications and vaccinations per protocol.
- Provide basic wound care and assist in other clinical procedures as needed.
- Educate patients on managing chronic conditions and general wellness practices.
- Perform routine screenings and health checks, identifying any changes that need escalation.
- Care Management
- Ensure compliance with CalAIM guidelines and Health Plans contract requirements delivering care management services according to organizational standards
- Manage a caseload of patients enrolled in the ECM program.
- Conduct comprehensive assessment of patients' health, behavioral and social needs using standardized tools.
- Develop, implement, and monitor individualized care plans based on the comprehensive assessment findings.
- Coordinate care transitions, including hospital discharges, ensuring patients receive appropriate follow-up care and support.
- Accompany patients to medical appointments and/or social services as deemed necessary.
- Community Engagement
- Establish trust and build relationships with patients to foster a supportive care environment and to promote self-management.
- Deliver patient education on preventive care and healthy practices.
- Assist in coordinating with local social services to connect patients to additional resources.
- Support outreach efforts by participating in community-based events and wellness programs.
- Documentation and Reporting
- Maintain accurate and timely documentation of patient interactions and clinical care in line with organizational and regulatory standards.
- Use electronic health record (EHR) systems to input data efficiently.
- Assist with patient data collection for program evaluation and reporting.
- Collaboration and Communication
- Work closely with Community Health Workers (CHWs), healthcare providers, and community organizations.
- Initiate/Participate in interdisciplinary meetings to ensure integrated care and share insights from the field.
- Communicate any significant observations or patient needs to the supervising RN or clinical lead.
Requirements
Minimum Qualifications
- Current California LVN licensure.
- 2+ years of clinical nursing experience, ideally in a community health, outpatient, or mobile clinic setting.
- Familiarity with local healthcare resources and social service agencies.
- Basic knowledge of public health principles and practices.
- Valid Driver's License.
Preferred Qualifications and Skills
- Experience working with unhoused or high-risk populations.
- Bilingual (Spanish-English) preferred.
- Knowledge of motivational interviewing, trauma-informed care, harm-reduction and other patient engagement strategies.
- Strong communication, organizational, and multitasking abilities.
- Proficiency in healthcare technologies, particularly EHR systems.
