Position Summary:
The Licensed Practical Nurse (LPN) PRN position at [Facility Name] is responsible for providing direct patient care under the supervision of a Registered Nurse (RN). The LPN will assist in maintaining a safe and therapeutic environment for patients.
Essential Duties and Responsibilities:
  • Patient Care:
    • Administer medications as ordered.
    • Perform treatments and procedures as directed.
    • Assist with patient activities of daily living (ADLs).
    • Monitor vital signs and report any changes to the RN.
    • Observe patients for signs of distress or deterioration and report findings to the RN.
  • Documentation:
    • Accurately document patient care activities in the medical record.
    • Complete incident reports as necessary.
  • Communication:
    • Communicate effectively with patients, families, and other healthcare team members.
    • Participate in patient care planning and coordination.
  • Quality Improvement:
    • Assist in implementing quality improvement initiatives.
    • Adhere to facility policies and procedures.
Qualifications:
  • Current LPN license in good standing.
  • Experience in a skilled nursing facility preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management skills.
