Job Summary:

Under the direct supervision of the assigned Supervisor, the Family Support Counselor has responsibility for providing support services to individuals and families as identified in the designated program and/or individualized plan. Services apply to youth (0-21 years old) and/or consumer/clients in community-based programs.

Essential Functions:

1. Provides individual and family support services in accordance with the individualized plans. Services may include: behavioral interventions and management, providing referrals, advocacy and support to individuals and families (clients), linkage and case management services, support groups, educational services and support, home management assistance, recreational activities, etc.

2. Assumes responsibility for configuring and facilitating the Child and Family Teams; coordinates, schedules, and facilitates Child and Family Team meetings for the development of individualized service plans.

3. Able to effectively engage and develop relationships with appropriate boundaries with assigned clients.

4. Effectively manages a flexible community-based schedule.

5. Completes all required documentation according to Agency and site standards within required time constraints. These may include: client contact forms, CANS, service activity summaries, data tracking, daily progress notes for all mental health services provided, mileage documentation, and other administrative duties, etc.

6. Is an active member of assigned treatment teams. Assumes assigned responsibility within the team in accordance to site needs which may include operating effectively within a Teaming Model and act as a liaison with other community agencies and resources.

7. Meets Agency service percentage standard reflecting expected time spent with clients, if applicable.

8. Completes other duties as assigned.

Minimum Required Education and Experience:

Must possess a high school diploma/GED and experience working with children, youth, adults and/or families.

Preferred Education and Experience

Bachelor's degree in psychology or in a related field is preferred.
Experience working with children (0-21 years old) and/or families is preferred

Position/Program Requirements:

Must possess a valid California driver's license, personal automobile insurance and clean driving record.
Must be physically and mentally fit. Must be willing to complete a Tuberculosis (TB) and drug screening test.
Must be willing to complete a personal background investigation conducted by the State of California.
Must be flexible to work nights and weekends, may need to work a split-shift, varying schedule according to operational needs.

Physical Requirements:

Must be able to work in an office or outdoor environment including standing, walking, and running, sitting for extended periods of time, bending, twisting, reaching, balancing, occasional carrying and lifting up to 25 pounds and up to 10 pounds frequently.
Must be physically able to perform Pro-Act Restraint Techniques, CPR and First Aid as trained.
Must be able to sit for prolonged periods of time in a vehicle for traveling up to 100 miles driving distance.

Education
Preferred
Bachelors or better in Psychology or related field
