Adult Family Care helps support families caring for a loved one in their home. Join a team of nurses who provide case management nursing, that focuses on care coordination and service navigation. This unique nursing positions offers flexibility around scheduling to meet both the staff and family’s needs. Conduct routine home visits based around family’s needs.
Complete all required documentation on individuals assigned, including health status reports, clinical assessments, and routine case notes.
Conduct routine training for staff and families.

Licensed Registered Nurse with two years’ experience in field of disabilities or working with the elderly.
Current license in the State of Massachusetts.
Valid driver’s license & Insured vehicle
