Mt. Carmel Rehabilitation and Nursing Center is seeking a dedicated full-time MDS Coordinator (RN) to join our Nursing Administration Team. Job Responsibilities:
RAI/MDS Expertise: Possess in-depth knowledge of the RAI/MDS process, including person-centered care planning, Medicare PDPM reimbursement, scheduling, month-end triple check, appeals process, and Medicare certification.
Assessment Coordination: Maintain accurate PDPM/Medicare and Medicaid/Private assessment schedules, ensuring proper reference dates for distribution to therapies, dietary, social services, activities, and nursing.
Education and Training: Provide ongoing education and training to staff on MDS and person-centered care planning.
Regulatory Compliance: Ensure compliance with RAI processes, including MDS assessments, Triggers and CAAS, and Care Planning, in accordance with state, federal, and OBRA regulations.
Medicare Certification: Manage the Medicare certification/recertification binder to meet Medicare physician certification requirements for Part A & B services.
Nursing Practices: Demonstrate proficiency in current nursing practices and procedures relevant to Long Term Care settings.

Requirements:
· Registered Nurse (RN) with a valid New Hampshire license
· RAC-CT certification preferred
· Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning
· Must have strong clinical and communication skills
· Extensive knowledge of the RAI process and reimbursement requirements for both federal and state
· Must be highly organized with the ability to multi-task throughout the day
· Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred
· Experience in developing templates to utilize in facility for audit/educational use
