The interventional pulmonary nurse navigator's primary job duties include supporting five Interventional Pulmonary providers across 3 locations:
Referrals
Triaging procedure referrals to prioritize them
Review referrals to determine if interventional pulmonary care is the appropriate sub-specialty
Reviewing images, but not required to interpret scans –
Deciding on the necessary procedure – based on algorithm
Identifying what testing is needed prior to procedures –
Obtaining records
Ensuring that pulmonary function tests, correct CT scans, and pre-operative assessments are completed prior to procedure (no decision on test ordering is required) –
Summarizing medical records for providers review
Identifying any gaps in care –
Reviewing procedure instructions and advising patient on medications prior to procedure
Educating patients and their families about interventional pulmonary care and addressing any concerns they may have prior to visit or after they return

Outpatient Registered Nurse Navigator / RN, Pulmonary
We are seeking a capable Registered Nurse Navigator/ RN who has acquired broad experience in caring for patients, and has developed a sound understanding about the care of a particular patient population.
Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship.
Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating, providing a link to research, serving as the primary point of contact for patients and families and providing indirect patient care.
Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.
MINIMUM QUALIFICATIONS:
Graduate of an accredited school of nursing; BSN required.
Two years nursing experience required, 1 year case management preferred. BLS required.
