Pulmonary Disease Navigator

JOBPURPOSE:
The Pulmonary Navigatorcoordinates and manages outcomes of a specific patient populationto facilitate achievement of quality, services, and cost. Providesone to one education, telephonic disease management services, andhome visits to individuals with pulmonary diseases. Assigned topatient caseload from initial inpatient admission ensuring a smoothtransitioning of care to post-acute care. Works collaboratively andmaintains active communication with physicians, Care Coordinatorsand interdisciplinary staff. Prepares, analyzes, and communicatesprofiles, reports, and summaries. Identifies and evaluates patientand family educationalneeds.

Qualifications:
MINIMUMEDUCATION REQUIRED:
Graduate of anaccredited school of Respiratory Therapy; Bachelors degree stronglypreferred.
MINIMUM EXPERIENCEREQUIRED:
Three years of recent andrelevant clinical experience related to respiratory care. Preferexperience with multidisciplinary teams. Experience with dataanalysis, Centers for Medicare/Medicaid Services, research, andprocess improvement.
MINIMUMLICENSURE/CERTIFICATION REQUIRED BY LAW: Currentrespiratory care practitioner licensed by the State of Georgiaunder the Composite State Board of Medical Examiners, holdRegistered Respiratory Therapist (RRT), credential from NationalBoard for Respiratory Care (NBRC). Credentialed as PulmonaryDisease Educator via American Association for Respiratory Care(AARC) within 6 months of hire. Basic Life Support (BLS)certification.
ADDITIONAL PREFERREDQUALIFICATIONS:
Advanced CardiopulmonaryLife Support (ACLS) preferred. Strong working knowledge ofhealthcare systems and community resources. Able to establishstrong working relationships within a healthcare system.
