James River Hospice is actively seeking a compassionate and highly committed PRN Hospice RN to join our growing team You will be responsible for the hospice case management of your assigned patients intermittently in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient

Brief Overview of Responsibilities for Registered Nurses:

Direct Care:

Perform comprehensive patient assessments and develop individualized care plans with the IDG.
Provide skilled nursing care and treatments as prescribed.
Educate patients/families on end-of-life care, healthcare decisions, and dietary counseling.
Conduct death visits and provide bereavement support as needed.
Administer medications and stay updated on drug therapies.

Case Management:

Coordinate all aspects of patient care, including services provided by IDG and contracted providers.
Ensure continuity of care by managing patient schedules and collaborating with team members to address emerging needs.
Keep patient care plans up-to-date and ensure documentation is accurate, complete, and timely
Supervise and evaluate care provided by Hospice Aides and Licensed Practical Nurses (LPNs), conducting supervisory visits as required

Qualifications of Registered Nurses:

Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience is required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.
