Job Summary:
We are seeking a detail-oriented individual to join our team. The ideal candidate will oversee and coordinate clients' long-term care, ensuring their needs are met efficiently and effectively as you will be gathering documentation & information. Communicating with a team as well as clientele having to multitask and be capable of managing their time & schedule.

Duties:
  • Reaching out to clients' outbound calls / Transferring calls as needed
  • Communicating via email, Phone calls/ Texts, or in person with clientele
  • Collecting important documentation
  • Asses information received
  • Coordinate accordingly with team members
  • Monitor clients' progress
  • Maintain accurate and up-to-date case notes and documentation
Experience
  • 2+ Years experience in Customer Service
  • Strong communication skills
  • Proficient computer skills
  • Demonstrate careful attention to detail
  • Previous experience in a similar role is preferred but not required
