Position Summary  We are seeking tech-savvy, LPN Patient Care Coordinators to conduct monthly telephonic outreach to a panel of chronically ill patients, developing and delivering personalized care plans that address their health concerns and challenges. We work in partnership with healthcare systems and practices to provide these services to their patient base and strive for care continuity and engagement. Essential Functions  
  • Manage and support patients’ healthcare needs via virtual phone conversations
  • Conduct and document visits leveraging technology platforms and EHRs
  • Develop personalized care plans to address patients’ physical, mental, and preventative health needs
  • Virtually coach patients to reach their care goals by following medical treatment plans that include healthy eating, physical activity, stress reduction, etc.
  • Connect patients with resources such as nutritionists, nurses, and doctors who can also assist them in meeting their care need
  • Prepare patients for appointments and procedures, and provide follow-up as needed
  • Coordinate some administrative aspects of medical record recovery, appointment management, and insurance claims
  • Review insurance plans and suggest tips to get the most from their benefit
  • Proactively suggest services or preventive screenings relevant to each patient
 Required Education and Experience  
  • Must have a Compact Nursing License that is clear and active
  • Must reside and work in the continental US
  • Must have a minimum of two years of clinical experience
  • Experience in care coordination or care management, health coaching, and motivational interviewing a plus
  • Experience and comfort with Electronic Medical Record
  • Experience using Microsoft Office products such as Teams, Outlook, SharePoint, and Excel
 Work Environment  
  • Equipment is provided
  • A wired, high-speed internet connection is required 
  • A private space free from outside noise and distraction, including noise and distractions from pets or children, and that complies with HIPAA privacy laws is required
 Physical Demands  This is a virtual position that requires you to sit or stand at a computer for 32-40 hours weekly and be able to make phone calls. Some equipment setup is required. Position TypeFull-Time Schedule

  • 30-40 hours weekly required
 Pay Structure  Orientation + Training (First eight weeks): $19.44 hourlyPost-Orientation: $22 hourly, plus a monthly bonus incentive. Our tiered bonus structure allows top performers to earn up to $525 in additional bonuses every month.  Referral Bonus Available
