The Community Health Worker (CHW), reporting to the Lead Care Manager will work directly with the Enhanced Case Management (ECM) team to identify eligible individuals and ensure their safe transition from skilled nursing facilities to community living when possible. The CHW will be cross-trained in all case management, transitions, and diversions programs to cover if needed. The CHW works as a member of the Choice in Aging (CiA) team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.

  • Work with older adults in Contra Costa County to help improve their quality of life by developing care plans, facilitating access to resources, advocating for client rights, and promoting self-sufficiency and empowerment.
  • Locate, contact, and engage members who have been identified as candidates using a progressive approach.
  • Document outreach and engagement attempts and modalities.
  • Attend and participate in monthly management meetings.
  • Participate in and/or direct functions related to fundraising and outreach.

Minimum Qualifications
  • 1. High school diploma or equivalent required, experience in case management with older and/or disabled adults; nonprofit experience preferred.
  • 2. Associate’s or Bachelor’s degree in related field preferred.
  • 3. Working knowledge of MS office, Google suite, and basic internet usage.
  • 4. Experience working with the Older Adult population preferred.
  • 5. Bi-lingual English/Spanish or English/Tagalog desired.

Job Type: Full-Time

Monday-Friday (Closed on all major holidays)

Hourly Rate: $21.00 - $24.00

Staff working Full Time (30+ hours per week) & Part Time (20+ hours per week) are Eligible for:

About Choice in Aging:

Choice in Aging (CiA) has been providing services to the community’s frailest people since 1949, when it was created by volunteers to serve children severely disabled by Polio. Over the years, the organization has provided a wide variety of programs, with the goal of promoting dignity and independence of people with disabilities and special needs. Our mission is to create opportunities where people can learn, grow, and age independently with dignity in community. Program History Adult Day Health Care (ADHC) is a program that was created in California in 1978 in response to a need to have an alternate solution to institutionalization. The program mirrors the skilled nursing staffing model, but is based in the community and typically operates during the day on weekdays. It is akin to a senior center for those who can no longer attend a senior center independently. It is especially beneficial for those who need additional support to do things like ambulate, eat, toilet, participate in activities and more.
