**Job Description:**

- Perform biopsychosocial assessments and develop a plan of care for patients and families.
- Gather data and formulate assessments using effective interviewing techniques, direct observation, consultation with the healthcare team, and medical record reviews.
- Revise the plan of care based on ongoing assessments of patient and family concerns, strengths, needs, clinical impressions, and specific spiritual and cultural values.
- Assess discharge planning resources and identify suspected situations of abuse and neglect.
- Provide direct clinical services, including discharge planning, individual and family counseling, crisis intervention, education and support group facilitation, and resource linkage.
- Establish therapeutic relationships recognizing diverse cultural norms and spiritual values.
- Communicate effectively to empower patients and families to develop productive coping mechanisms and thought processes.
- Address resistance and conflict effectively and intervene to decrease tension and assist with non-productive behavior evaluation.
- Demonstrate self-awareness and maintain professional boundaries to effectively intervene with patients and families.
- Provide psycho-education related to health literacy, transitions of care, and community resources.
- Educate interdisciplinary staff regarding high-risk situations and psychosocial aspects impacting patients and families.
- Advocate for patients and families to ensure services, access to care, and protection of safety and rights.
- Promote patient self-determination and assist teams in understanding patient perspectives.
- Advocate for culturally and linguistically competent care and address ethical issues appropriately.
- Document assessments, plans, interventions, and involvement in patient records accurately and timely.
- Maintain positive and effective relationships with other hospital departments, medical staff, and community representatives.
- Collaborate on process improvement activities and participate in staff and community education efforts.
- Foster an environment of mutual support and professional excellence within the department.
- Participate in department activities, provide guidance and consultation to interns and colleagues, and meet performance and competency standards.
