GENERAL DESCRIPTION- UNION POSITION- HYBRID:
Provides intake, screening, assessment, counseling, crisis intervention, case management and supportive services to individuals, families, and groups.
RESPONSIBILITIES:
- Conducts initial intakes and annual updates.
- Engage members in a welcoming, empathetic manner even during acute phase.
- Screen members for co-occurring mental health and substance use disorders.
- Demonstrate the knowledge of diagnostic criteria for psychiatric and substance use disorders.
- Provides therapeutic interventions to members and families during individual and/or group therapy and other settings.
- Provide crisis intervention as needed.
- Evaluates the progress of individuals on an on-going basis.
- Obtains information when appropriate from a variety of sources to develop a clinical history.
- Attends Interdisciplinary team meetings to review problems and plan for solutions.
- Assist individuals with identifying changes in behavior, attitudes, feeling and thought that are triggers for relapse. Assist in the development of a relapse prevention/crisis plan utilizing personal, community and provider support.
- Provide education and/or consultation to help individuals, families, and support persons to understand the interplay of mental health and/or substance use disorders, the phases of recovery, service options, self-determination, and the course of treatment. Clinicians will encourage participation in community and peer support groups.
- Communicate and collaborate with other external providers.
- Performs discharge planning activities in accordance with best practice standards for individuals being released from a hospital setting.
- Assists individuals with medication needs and coordinates with psychiatrist as appropriate based on treatment needs. Provide support, education, and reinforce the use of prescribed medications to treat substance use and psychiatric disorders.
- Responsible for authorizing medically necessary services for all individuals.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist individuals with trauma related issues.
- Provides clinical management of individuals and/or families in crisis and facilitates psychiatric hospitalization as needed.
- May perform case management functions and assist the case manager when needed to provide in-office services to members, including assessment, linking/coordinating, monitoring, reassessment, follow-up, resource development and advocacy.
- Ensures all actions, job performance, personal conduct and communications represent the agency in a highly professional manner.
- Advocate and provide additional guidance, support and continuity of care when meeting with members and their support systems.
- Develops IPOS treatment plans based on evidence-based treatment approaches, measurable objectives, standards of care, and professional practice guidelines.
- Ensures timely completion of required documents and assessments per agency requirements.
- Counsel’s members on interventions to modify behaviors associated with health risks; understands the correlation between physical and behavioral health issues.
- Refers members to internal and external supportive services, as medically necessary, or by members preference.
- Promotes the members independence, rehabilitation, and recovery, to reduce risks of becoming unhoused, inpatient hospitalization, and other negative outcomes.
- Maintains compliance with productivity requirements ensuring timely completion of documentation (within 24 hours of the kept appointment) and use thorough and accurate coding of visits.
- Participates in multidisciplinary care team(s) and identifies problems and provides feedback to team members regarding a solution; contributes to the integration of services inclusive of primary care, behavior health, dental services, and other supportive services; serves as a resource to other providers and staff.
- Demonstrates compliance with all stakeholders, Medicaid, CCH policies, procedures and state/federal regulations; takes part in performance improvement activities such as: quality assurance audits, continuing education, and peer reviews.
- Participates in meetings/conferences and continuing education and demonstrates a genuine desire to develop strategies for improved clinic operations and high-level patient care.
- Ensures state licensure and national certification remain current and valid.
- Ensure member’s recipient rights are acknowledged and maintained.
- All other duties as assigned by supervisor or as indicated by program or grant/funding source.
- Participates with all pertinent staff in the development, review and the delivery of treatment and advocacy.
- Provide periodic assessment/review of member’s needs, assist with developing person-centered treatment plans with member and supports. Evaluate member’s progress on an ongoing basis and completes discharge summaries.
- Maintains all clinical documentation pertaining to diagnosis, treatment, and delivery of services.
- Acts as liaison between other agencies, hospitals, legal authorities, caretakers, as well as other community partners/stakeholders.
- Participates in in-service training, clinical supervision, case staffing, consultations, and other meetings.
- Participate in community events.
- Supervises students and/or volunteers as mutually agreed upon.
- Performs other duties as assigned.
EDUCATION AND EXPERIENCE:
- Master’s degree in social work, psychology or counseling required.
- Current State of Michigan licensure in one of the following: LMSW, LLMSW, LP, LLP or PC required.
- One year of experience in treating or working with a person who is exhibiting mental illness, substance abuse, and/or co-occurring behaviors preferred.
- Must be willing to complete requirements for a Michigan Certified Board of Addiction Professionals (MCBAP) Development Plan (Certified Alcohol and Drug Counselor, CADC, or Certified Advanced Alcohol and Drug Counselor, CAADC) within the first 90 days of hire.
- Leading to certification and are timely in its implementation or are functioning under a time-limited exception plan approved by the regional PIHP. (applies to SUD Therapists only)
- Valid Michigan Chauffeur’s Driver’s License required; Candidates who do not have a chauffeur's license, may still apply but must acquire a chauffeur’s license within 30 days of employment. Demonstrated ability to adhere to State of Michigan driving laws, evidenced by maintaining an insurable driving record in accordance with CCH commercial auto insurance carrier required. (applies to ACT and Housing Therapists only)
- For therapy codes: Mental Health Professional [Mental Health Code, Section 330.1100b(15)] - An individual who is trained and experienced in the area of mental illness or developmental disabilities and who is one of the following: a physician, psychologist, registered professional nurse licensed or otherwise authorized to engage in the practice of nursing under part 172 of the public health code (1978 PA 368, MCL 333.17201 to 333.17242), licensed master’s social worker licensed or otherwise authorized to engage in the practice of social work at the master’s level under part 185 of the public health code (1978 PA 368, MCL 333.18501 to 333.18518), licensed professional counselor licensed or otherwise authorized to engage in the practice of counseling under part 181 of the public health code (1978 PA 368, MCL 333.18101 to 333.18177), or a marriage and family therapist licensed or otherwise authorized to engage in the practice of marriage and family therapy under part 169 of the public health code (1978 PA 368, MCL 333.16901 to 333.16915).
- For intake and case management services: Qualified Mental Health Professional (QMHP) - Individual with specialized training3 (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.
National Health Service Corps Loan Repayment Program:
Full Time Clinical Guidelines
- Clinicians must work a minimum of 40 hours per week, for a minimum of 45 weeks service year.
- At least 32 hours per week must be spent providing direct patient care at the approved service site(s); of the minimum 32 hours spent providing patient care, no more than 8 hours per week may be spent in a teaching capacity.
- The remaining 8 hours per week may be spent providing patient care for patients at the approved site(s), providing patient care in alternative settings (e.g., hospitals, nursing homes, shelters) as directed by the approved site(s), or performing clinical-related administrative activities (limited to 8 hours/week); clinical-related administrative activities may not exceed a total of 8 hours per week.
Part Time Clinical Guidelines:
- Clinicians must work a minimum of 20 hours per week, for a minimum of 45 weeks service year.
- At least 16 hours per week must be spent providing direct patient care at the approved service site(s); of the minimum 16 hours spent providing patient care, no more than 4 hours per week may be spent in a teaching capacity.
- The remaining 4 hours per week may be spent providing patient care for patients at the approved site(s), providing patient care in alternative settings (e.g., hospitals, nursing homes, shelters) as directed by the approved site(s), or performing clinical-related administrative activities (limited to 4 hours/week); clinical-related administrative activities may not exceed a total of 4 hours per week.
