- Conduct assessments and develop individualized care plans for patients
- Carry out all steps of the nursing process while involving the patient/family and other members of the care team to help patients meet expected outcomes
- Administer medications and treatments as prescribed by the supervising provider
- Maintain accurate health records using our electronic health record system
- Organize and provide education in our counseling and support programs
- Provide culturally competent care that comprehensively considers the physiological, psychological, and spiritual aspects of health
- CA LVN license
- BLS or CPR certification
- Ability to clear a background check and drug screen
- 2+ years of LVN / Licensed Vocational Nurse experience
