Job Purpose:The Integration Coordinator is responsible for managing and overseeing the integration of various systems and processes within the organization. This role ensures that different components work together seamlessly to enhance operational efficiency, improve service delivery, and achieve organizational goals. The Qualification Specialist reports to the Qualification Team Lead and the Associate Director of Operations.

Personal Attributes:Detail-oriented, proactive, and able to work under pressure. Demonstrates the ability to manage multiple tasks simultaneously and effectively.

Key Responsibilities:
  • Technical Support: Provide technical assistance and guidance during the integration process. Troubleshoot and resolve issues to ensure smooth and successful integration.
  • Monitoring and Follow-Up: Regularly monitor clients’ progress and service utilization. Provide feedback and coaching to clients as needed.
  • Documentation: Maintain accurate and up-to-date case records.
  • Communication: Facilitate effective communication between clients, the internal team, and providers. Ensure all parties are informed and involved in the integration process.

  • Strong organizational and time management abilities.
  • Excellent analytical and problem-solving skills.
  • Capability to work independently and collaboratively within a team.

Integration Tasks:
  • Set up physician and note categories.
  • Coach staff on utilizing the integration for consultations and notes.
  • Follow up on patients not mapped through the integration.
  • Request deactivations for facilities that are no longer serviced or have changed ownership.
  • Troubleshoot support cases
  • Provide activation instructions for pending enrollments.

Experience:
  • Preferred Experience working with Skilled Long-Term Care facilities.
  • Intermediate proficiency with PC and Windows applications
  • Proficiency in software such as Microsoft Office and Excel is required.
  • Preferred experience with Point Click Care software and Matrix EHR software.
