As a Care Manager in ACE’s Prevention program, you’ll provide comprehensive support to participants and their families, guiding them through every stage of the program. Your responsibilities will include community outreach, intake and enrollment, individualized and family case management, and leading individual and group activities that foster growth and resilience. With a focus on thorough documentation and effective service delivery, this role is ideal for someone dedicated to making a positive impact within the community. The Prevention program supports youth ages 10-15 through family engagement and personal development. Our mission is to build resilience in youth, families, and communities through mentorship and collaboration. Graduates gain the tools to navigate life’s challenges and overcome systemic barriers.

DUTIES AND RESPONSIBILITIES:
  • Participate in cultural and community-building activities/events.
  • Works to incorporate into organizational culture best practices for diversity, equity, inclusion, restorative justice, and trauma-informed care. 
  • Performs outreach and marketing duties to recruit participants and educate the community on ACE Services.
  • Carries out intake and eligibility determinations and makes appropriate referrals when the applicant is not eligible for ACE services; conducts participant assessments leading to enrollment; completes and updates participant service plans. 
  • Arrange and participate in participants’ family meetings, individual meetings, team meetings, case consults with GRYD Intervention, etc., and coordinate wrap-around services with other providers; provide transportation of program participants to meet program goals.
  • Organizes, prepares and facilitates intentional youth development and skills-based workshops for participants.
  • Enters data into participant databases (organizational and program); assists in the writing of periodic program reports.
  • Provides case management for an average standing caseload of twenty-five participants.
  • Responsible for administering eligibility assessment.
  • Responsible for inputting participant information and case notes into appropriate database(s)
  • Responsible for assisting participants and their families in making referrals to partner organizations.
  • Responsible for organizing celebration activity meetings with participants and support systems.
  • Participates in all GRYD community-based events including Saturday Night Lights and Fall Friday Night activities.
  • Conducts court advocacy as needed for program participants. 
  • Plan and coordinate Field trips and provide onsite supervision. 
  • Address and hold youth accountable for program expectations through a trauma-informed lens.
  • Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Possesses at least moderate knowledge of the role and duties of the position; understands the department’s roles and is able to meet the expectations of the position for work schedule, punctuality, attendance, time management, productivity, and quality of work.
  • Operates as a reliable, flexible team player who works well with minimal supervision; has a good work ethic; is able to work effectively with team members demonstrating creativity, initiative, adaptability, and the ability to accept accountability and critical feedback.   • Possesses the technical and software skills to perform the duties of the position (including Google Suite and/or Microsoft Office).
  • Possesses the interpersonal skills needed to promote a safe, positive, and inclusive professional work environment conducive to effective service delivery; communicates effectively both orally and in writing. 
  • Demonstrates cultural competence/cultural humility in community relations and service delivery.
  • Is sensitive to community, cultural and personal dignity of program participants and is able to successfully engage, assess, and plan services for participants typical of the program’s target population. 
  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families and the need to maintain professional standards and practices. Is familiar with standard approaches to accurate participant record keeping, including timely charting, notation formats, content limitations, and confidentiality practices.
  • Possesses knowledge of programs, regulations, and procedures regarding human services, social services, justice, and family case management?
  • Knowledge of community/gang dynamics within the GRYD Zone 
  • Able to operate within ACE’s designated GRYD zone, exposing oneself to gangs found within the zone.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical discomfort or distress.

PHYSICAL REQUIREMENTS:
  • Prolonged periods of standing and walking throughout facilities.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must be able to bend, lift, stretch, climb, and crawl to provide services.

QUALIFICATIONS:
  • Bachelor’s Degree in a relevant field and 2+ years of experience working with youth; or an Equivalent combination of training and experience.
  • Spanish language skills, verbal and written preferred. 
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance. 
  • Must submit to a Department of Justice Criminal background check. 
  • Initial TB employment clearance required.

    • Vacation will then accrue to two weeks vacation after completion of 2 years working at ACE and three weeks of vacation time after 5+ years of employment.

  • 15 paid holidays (selected by ACE).

The anticipated pay range for candidates who will work in California is $28.85 to $30.45 hourly.  The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
