Responsibilities: 
As the MDS Nurse Coordinator, your role will encompass the following key responsibilities:
  • Oversight of scheduling, completion, and accuracy of all Minimum Data Set (MDS) item sets, ensuring adherence to regulatory requirements.
  • Coordination of the development of a comprehensive written plan of care for each resident, guaranteeing the establishment of realistic and measurable goals.
  • Chairing resident care plan conferences and effectively communicating concerns to the Administrator.
  • Ensuring residents, families, and responsible parties are promptly contacted for care plan conferences in accordance with Resident Assessment Instrument (RAI) guidelines.
  • Planning, conducting, and participating in In-service Education Programs relevant to the RAI process.
  • Assisting in the implementation and maintenance of an efficient orientation program for new employees.
  • Monitoring residents' medical records for consistency, content, and addressing any discrepancies identified.
  • Maintaining up-to-date ICD-9 codes for current diagnoses.
  • Reporting IT concerns to the help desk in a timely manner to ensure uninterrupted workflow.
  • Actively participating in the development, maintenance, and implementation of the facility’s Quality Assessment and Performance Improvement (QAPI) program.
  • Attending relevant meetings as directed by the Administrator.
  • Performing any other related duties as assigned by the management.

Qualifications:
To be considered for this position, candidates should possess the following qualifications:
  • Current, unencumbered RN or LPN license in the state.
  • Current CPR certification or ability to obtain within 90 days of employment.
  • Working knowledge of OBRA, IDPH, IDPA (for Illinois homes), and Wisconsin DQA (for Wisconsin homes) standards.
  • Proficiency in the English language, encompassing reading, writing, and speaking.
  • Ability to communicate tactfully and effectively with residents, family members, visitors, personnel, government agencies/personnel, and the general public.
  • Demonstrated leadership ability and willingness to collaborate harmoniously with personnel.
