We are currently searching for a compassionate and empathetic Hospice Team Coordinator to join our team. Your primary focus will be to support individuals and families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life. Please consider joining our growing team and becoming a part of our caring team.

Responsibilities:
  • Scheduling of Aide visits.
  • Assist with field visits as needed.
  • Collaborate with Admissions Coordinator in patient referrals and admissions.
  • Confer with physicians and clinical team.
  • Provide support with functions regarding auditing and compliance.
  • Update and maintain patient medical records for facilities.
  • Ensure proper documentation in a timely manner.

Qualifications:
  • Certified Nursing Assistant (CNA)
  • Hospice experience preferred.
  • Strong organizational skills.
  • Effective communication skills.
  • Successful completion of Background Screening.

Job Type: Full-time

Salary: $25.00 - $28.00 per hour
