Key Responsibilities:

  • Assess health status through obtainment of relevant health history and comprehensive physical exam, preventative screening, and identification of medical risk factors.
  • Formulate appropriate differential diagnosis and identification of individual, family, and community needs.
  • Develops treatment plan to maximize the health potential of the individual, family, and/or community.
  • Orders and adjusts medication per collaborative written agreement and prescriptive authority in accordance with the Indiana Nurse Practice Act, or in consultation, and with signature of physician.
  • Directs the development, implementation, and evaluation of standards for nursing practice.
  • Serves as a clinical resource for the nursing staff, physicians, hospital, and other healthcare providers.
  • Demonstrate knowledge of growth and development over the lifespan.
  • Provide prompt and professional medical attention to patients.
  • Maintains documentation in accordance with IU Health policies, standards, and regulatory guidelines.
  • Expands and maintains professional knowledge through continuing medical education.
  • Perform other duties as assigned.

Qualifications:

  • FNP or PA certification required
  • 1+ year of primary care NP or PA experience required
  • Must be comfortable working with all ages

Hospital Overview

Delivering on our vision to make Indiana one of the nation s healthiest states, Indiana University Health expanded into the northeast Indiana market in 2018. Currently, IU Health operates four primary care clinics, two urgent cares, and a radiology center in the Fort Wayne area.
