Clinical Nurse Specialist

The Clinical Nurse Specialist (CNS) is responsible for assisting in the delivery of quality clinical services within an assigned group of patients. The CNS will review and deliver standardized, comprehensive, and effective training for all assigned caregivers.
The CNS will represent Family First Homecare and its services in a competent, professional, and responsive manner while maintaining standards of high-quality patient care and customer service in accordance with all state, local and federal requirements, rules, and regulations.
KEY RESPONSIBILITIES
- Provide input to the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding Care givers
- Demonstrates clinical judgment in responding to priority patient needs
- Provides training, guidance, and mentoring for all assigned caregivers
- Collaborates with the Family First leadership teams and the compliance committee in identifying, developing, and the delivery of consistent clinical information
- Reviews assigned patient's appropriateness for homecare
- Communicates with multidisciplinary team
- Demonstrates effective communication with the patient / family while maintaining an appropriate nurse-patient relationship
- Ensures a current physician orders, plan of care/ care plan is place for all assigned patients
- Ensures that the competency program is followed and provides educational support for the clinicians as needed for skills development
- Promotes employee engagement and retention
- Demonstrates ability to manage job related stressors and constructive feedback during intra/interdepartmental relations
- Participates in the development and review of policies and procedures to ensure compliance with all State, Federal, and Accreditation standards
- Identifies clinical and quality metrics trends and ensures that appropriate departments are aware
- Partners with the DON/DOO/VPC to educate cohesive teams toward common goals
- Assists management with the development and implementation of new products and services
- Maintains professional, supportive, and responsive interpersonal communication skills
- Performs other duties as assigned
REQUIRED
- RN license required
- 2 + years of related experience as an RN
- Previous pediatric experience
- Excellent verbal and written communication skills
- Demonstrated ability to effectively teach
- Demonstrated ability to foster a collaborative, interdisciplinary environment that supports quality care/service and staff empowerment
- Self-directed individual with high organizational and priority setting abilities.
- Strong computer skills to include Microsoft Office Suite
- Working knowledge of home care regulatory and federal requirements
- Previous supervisory experience (any profession)
- Position is stressful in terms of meeting deadlines.
- It is primarily a desk job which essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication.
- It requires minimal lifting of office records and printouts.
- Travel may be required by car or airplane to local, out-of-town, or state seminars, conferences, or meetings.
- Must ability to read 12 point and larger type, communicate clearly and be able to hear adequately on the telephone with no more than an amplifier.
