North Suffolk Community Services is looking for a Family Partner to join our Community Treatment Team (CTT) Program. The Family Partner provides an important role as a team member on the CTT Team , and delivers peer-to-peer education, coaching, modeling, and guidance to a Parent/Caregiver of an individual diagnosed with a SMI and ASD. The position is a paraprofessional that requires personal experience as a parent, caretaker or guardian of a person with Autism Spectrum Disorder (ASD) and Serious Mental Illness (SMI).

This position is Monday-Friday; 20 hours per week. Hours are to be determined.

What you’ll do:

  • Provides parents and caregivers with information about the NS CTT Service and the wraparound process and resources, including the Family Partner role to assist them to successfully engage.
  • Participates in CTT assessment, engagement, and ongoing service provision to ensure access, voice and choice within the wraparound process and to support the parent/caregiver’s connection to the ESP and community providers, as necessary.
  • Assists the Parent/Caregiver in meeting the youth’s needs by educating, supporting, coaching, modeling or guiding. These activities are delivered in the Parent/Caregiver’s home and community.
  • Partners with Parent/Caregiver to develop and create realistic indicators for the Transition Indicator tool.
  • Shares lived experience with Parent/Caregiver to build rapport, engage and inspire the Parent/Caregiver towards improving parental care of youth.
  • On-call responsibilities as needed.
  • Performs other related duties as assigned.

What you’ll need:

  • Education:
    • Associate Degree in a Human Services field AND 1-year of experience with target population required.
    • Bachelor’s degree preferred OR
    • High School diploma/GED and a minimum of 2 years’ relevant experience.
  • Experience:
    • Experience working with parent/caregivers and youth with Serious Mental Illness (SMI) and Autism Spectrum Disorder (ASD) and navigating child and family-serving systems.
    • Lived experience as a caregiver, guardian or parent of a youth or individual with ASD and SMI Diagnoses
  • Skills:
    • Driver’s license and reliable transportation required.
