JOB SUMMARY

Under the general supervision of the Administrator, the MDS Coordinator will be responsible for conducting and coordinating the development and completion of the resident’s assessment (MDS) in accordance with the requirements of this state and the policies and goals of Glencroft. Must be able to work with adult and geriatric patients that are chronically ill and/or mentally disoriented.


PRIMARY ACCOUNTABILITY

A thorough knowledge of the practices and procedures in the use and function of MDS Coordinator principles. An MDS coordinator is responsible in managing a nursing team in a medical facility. MDS coordinators serves as liaisons in the intake and evaluation process of patients in a skilled nursing facility, where they're responsible for up-to-date assessments of the patient and oversee the interdisciplinary assessment for all skilled nursing patients. The MDS coordinator monitors patient care by assessing procedures, speaking with patients and recording medical codes. MDS coordinators also enter medical codes and are involved in medical billing processes, ensuring they maintain accurate medical records. Strong written and oral communication skills. Effective in establishing and maintaining rapport with staff, residents, resident's families, and visiting doctors. Must possess the ability to maintain composure under pressure. A professional appearance is to be maintained at all times. The MDS Coordinator will possess a genuine interest in and concern for geriatric, chronically ill, and/or disabled persons. Must be able to fluently communicate in or to take or give direction or guidance in the English language.


ESSENTIAL FUNCTIONS

1.    Regular attendance is considered an essential function of this position and is necessary for the efficient operation of the business.

2.    Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including implementation of the raps and triggers.

3.    Coordinates the development of a written comprehensive plan of care by all disciplines for each resident that identifies the problems/needs of the resident, indicates the care to be rendered, goals to be accomplished, and which discipline is responsible for each element of care.

4.    Ensures that the care plan includes measurable objectives and timetables to meet the resident’s medical, nursing, mental, and psychological needs as identified in the resident assessment.

5.    Ensures that an initial assessment is completed within 14 days of admission and that the comprehensive care plan is completed within 21 days of admission and review occurs at a minimum every 90 days thereafter.

6.    Confers with and assists Health Information Coordinator with chart audits relating to resident assessments to ensure compliance and correct coding of MDS.

7.    Participates in facility surveys (inspections) made by authorized government agencies and consulting companies employed by this facility. Assists Administrator and Staff Development with training new employees on the MDS process to include the importance of each area of documentation relating to the resident assessments.

8.    Conducts both test and final transmission process of resident assessments to State as required by regulations. Assists HIM Coordinator in reviewing test transmission and corrects all errors prior to final transmission to State. Maintains all verification of transmittals in notebook in organized manner.

9.    Provides resident assessment (MDS) reports to VP of Health Services and the Health Information Coordinator upon request in a timely manner.

10. Meets with other department supervisors or staff concerning the resident assessment/care plan functions on an as needed basis. Maintains an effective and friendly working relationship with all health professionals, physician’s consultants, and government agencies that may be involved in resident assessment care plan functions.

11. Assists all disciplines with understanding the MDS process and notify them of any discrepancies in charting that may conflict with the completion of the MDS.

12. Ensures that all members of the assessment team are aware of the importance of the accuracy and completion of their assessment functions and that they are aware of penalties, including civil money penalties, for false certification of information on the resident assessments (MDS).

13. Coordinates the review and revision of resident assessment/care plan every 90 days after the initial assessment is completed and assures that they are evaluated and revised each time an assessment is done or when there is a change in resident status.

14. Ensures each portion of the resident assessment (MDS) is signed and dated by person completing that portion of the MDS.

15. Assists the VP of Health Services and the Health Information Coordinator and other relevant Directors and Supervisors in maintaining good rapport and morale between and within departments to ensure that a team effort is achieved in developing and implementing comprehensive assessments and care plans for each resident.

16. Serves, participates, and attends various committees of the facilities as required, or as requested by VP of Health Services or Health Information Coordinator.

17. Attends and participates in continuing education programs designed to assist in professional development.

18. Possesses ability to make independent decisions, possess excellent leadership and supervisory abilities, and willingness to work with and train other personnel.

19. Possesses the ability to simultaneously address multiple demands and issues and make correct decisions in a busy operational environment and seek out new methods and principles and be willing to incorporate them into existing nursing practices.
