A full-time Post-Acute Liaison's primary responsibilities include clinical screening and assessment of the potential patient/resident, assisting in the facility admission process and ensuring a smooth transition of the patient to the next level of care. While managing patient placements from the hospital setting to one of our Skilled Nursing & Therapy Centers, they will also work to partner/network with other health care professionals.
Responsibilities
- Develop strong business relationships with key hospital referral personnel, including Case Managers and physicians.
- Networking and business development with hospitals, LTACs and physicians groups to provide a consistent source of transitioning patient referrals.
- Perform pre-admission patient screening/assessment, including clinical, equipment and transportation needs.
- Work in a collaborative manor with the facility Admissions Coordinator to create an exceptional patient admission experience.
- Serve as an educational resource for patients, families, hospitals and healthcare professionals
- Responsible for administrative and procedural requirements relating to territory accounts; including recording daily activity within the referral system.
Qualifications
- Must be a licensed nurse (RN or LPN)
- Experience as a hospital liaison preferred
- Experienced in Skilled Nursing or Skilled Rehabilitation environment
- Clinical Sales or Business Development experience
- Reliable transportation required
