Community Connector ASL

Scope:

This position will provide safe, healthy and enriching interactions for children served through SKSF Community Connector Program. This position assists the agency professional staff and is under the direct supervision of the Foster Placement Manager and Foster Care Placement Director.

Duties:

• MUST be ASL FLUENT
• Must be able to work independently in the community and facilitate community participation and activities.
• Preferred: Experience in behavioral training and ability to be assigned to working with participants with high behaviors exhibiting the following: aggression, property destruction, elopement and self-injurious behaviors.
• Ability to implement established goals and treatment as determined in the participant's individual plan, protocols and behavior plan. Adhere to all person-centered planning while maintaining confidentiality.
• Complete and maintain necessary paperwork, incident reports in a timely fashion, and documenting their activities and goals on a weekly basis.
• Respect and promote the rights of the client.
• Provide monthly activity calendar, in advance, for participants in the program with activity destinations and anticipated mileage for approval by the Program Director.
• Attend and participate in staff meetings, meet and greets with potential clients and their family, and complete all required training. Maintain training requirements to include behavior management and techniques.
• Provide for the transportation needs of the participants using company van or personal vehicle (prior approval needed for personal vehicle). Maintain vehicle maintenance and documentation as required by SKSF policies for transporting participants.
• Provide integrated supports to the client, including facilitating relationship building.
• Participate in the implementation of the Individual Service and Support Plan.
• Promote awareness of the abilities of the individuals with disabilities, while maintaining confidentiality.
• Document and immediately report any incidents of concern to the Foster Care Placement Manager and Placement Director.

• Must be ASL fluent.
• Minimum of a High School education or equivalency. Would prefers an Associate’s Degree in Child Welfare or other Human Services Field.
• Experience with general administrative and recordkeeping practices.
• CPR Certification, First Aid and Standard Precautions within 30 days of hire.
• Current physical within the last 2 years per State of Colorado's rules and regulations.
• Must be able to lift and carry children as needed (able to lift 501bs.).
• Valid Driver's License, current vehicle insurance and an acceptable driving record per SKSF standards.
• Able to drive a vehicle safely while transporting passengers.
