Job Title: Referral Navigator
Reports to: Team Leader-Referrals
FLSA Status: Non-exempt
Date: January 2022
Position Summary:
The Referral Navigator will obtain appointments for patients requiring specialty medical care and/or testing not provided at the Center. This position requires face-to-face interaction with patients while coordinating times, dates and financing for specialty appointments or testing deemed necessary by the medical provider.
This position may have remote work requirements.
This position is subject to all current CMS vaccine mandates.
Principal Duties and Responsibilities:
- Meeting with patients on a daily basis acting as intermediary to obtain dates and financing for specialty services by contacting specialty offices and setting appointments
- Providing medical records, request forms or orders to specialty providers via fax or other method to provide an accurate and timely record of the patients past medical history, medications, and reason for referral
- Educating patients regarding basic requirements for testing procedures such as CT scans, plain film studies, and MRIs
- Track appointments/referrals made via EMR and record them in the proper locations.
- Works with Case Managers to assist patients with travel, counseling and other special needs
- Promotes Patient Centered Medical Home and contributes to the development of policies and procedures
- Works with PCMH to provide individualize assessment of patients educational needs and provide the knowledge and tools so the patients can manage their health effectively
- Participates in all required PHCC, OSHA and Emergency Preparedness training
- Follow up with outside providers to ensure patient compliance with appointments
- Participates in helping the provider meet all meaningful use criteria
- Receive all testing results for purposes of tracking
- Demonstrate initiative, ability to work with others, professional judgment, and effectively perform other duties as assigned
Required Knowledge, Experience, Skills and Licensure/Registration:
Working knowledge of Microsoft Excel and Word programs
Working knowledge of basic medical terminology
Working knowledge of medical office processes and ability to maintain strict patient confidentiality
Referral experience/medical office experience desirable.
Working Conditions:
The Referral Navigator will spend the majority of his/her time in an ordinary medical office environment and will have direct patient contact.
While performing the duties of this job, the Referral Navigator is regularly required to sit; use hands to finger, handle or feel; reach with hands and arms; and talk and hear; stand and walk, and occasionally required to climb or balance and stoop, kneel, crouch or crawl; occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distant vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
