Duties & Responsibilities:

  • Maintain access to all electronic systems of record needed to obtain data to provide a variety of medical and dental readiness reports.
  • Provide guidance and assistance to Soldiers and commanders regarding medical and dental readiness statuses.
  • Monitor the status of health assessments within the organization and ensures appropriate documentation and update in hard copy medical records and electronic systems of record.
  • Possess knowledge of medical terminology and Army Regulation (AR) 40-501 and AR 40-502.
  • Coordinate and ensures the timely and accurate input of medical and dental readiness data.
  • Organize and research patient records, extract needed information, and review records for completeness, accuracy, and consistency within established guidelines.
  • Review, validate, and forward requests for temporary and/or permanent medical actions to include profile determination and medical boards.
  • May assist with a variety of administrative/clerical support functions to ensure the effective operation of the office.
  • Maintains a variety of files, records, and forms; prepares special and recurring reports; receives and distributes mail and composes replies to routine military and non-military correspondence; maintains suspense records of items requiring action or replies and notifies staff members when action is required.
  • Responsible for tracking medical status of ALL assigned Soldiers.
  • Responsible for medical charting in eCase.
  • Request profile exams/evaluations.
  • Request audiology exams/evaluations.
  • Maintain automated referrals indicated in MEDCHART.
  • Submit case processing, medical documentation, and profile concerns to military/civilian provider.
  • Attend PHA/SRP/MDRE events as determined by the needs of the state and the individual units.
  • Ensure those Soldiers referred by a provider are followed by the case management team.
  • Manage Soldier's care from the identification of the condition to when the Soldier is either:
    • Considered medically ready.
    • Administratively separated from the military.
  • Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the Army Regulation 40-501, Army Regulation 40- 502, and other applicable guidance.
  • Ensure all medical documentation received from the Soldier and/or their medical team is received and placed in appropriate electronic record, as well as the Soldier's hard copy record.
  • Maintain Soldier medical records in accordance with Army Regulation (AR) 40- 66.
  • Provide Records Custodian or delegate with all relevant medical and dental documentation.
  • Obtain DD Form 2870 Release of Information (ROI) from Soldier.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
  • Review and track health assessment (PHA, SRP, MDRE) lab results, contact SM, recommend follow on care from a military or civilian provider.
  • Upload medical documents of case managed Soldiers into Health Readiness Record (HRR). Stamp all medical documents for verification after uploading.
  • MODS entry of immunizations, designation of pregnancy, eye exams, medical warning tags, and all applicable medical and dental readiness updates.
  • Provide all medical documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the contractor to the government. Copy of each supporting transfer document shall be provided to the COR at the end of each month no later than 7 calendar days at the end of the month.
  • Contractor will ensure all administrative charting in eCase is completed.
  • Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
  • Update eCase with any communication with SM regarding their condition. (email, voice call, or personal interview).
  • Communicate with delegated government personnel to track compliance versus noncompliance.
  • Run MEDPROS report Pre and Post PHA/SRP/MDRE event.
  • Monitor MEDPROS to ensure all scheduled services are completed/updated and current medical status is reflected.
  • Report Medical Readiness to the designated Medical Readiness Detachment representative, Deputy State Surgeon, or State Surgeon.
  • Ensure Soldiers with T3, T4, P3 or P4 profiles are case managed in the eCase module.
  • Provide supporting documents to Health Support Services as needed.
  • Other medical and dental readiness duties as assigned.
  • Maintain a clean, professional, and organized workspace.

Qualifications:

  • High school diploma or equivalent.
  • Computer proficiency, specifically with Microsoft Word and Excel.
