SUMMARY:

NACC’s mission is to promote the health & wellness of mind, body, and spirit of Native American families. We offer a full range of healthcare services that include Medical, Behavioral Health, Dental, and Substance Abuse. This position provides comprehensive medication management (CMM) services to ensure optimal patient care and education in regards to medications, ensuring medications are indicated, effective, safe, and convenient.

DUTIES AND RESPONSIBILITIES:

Essential Functions:

  • Provides comprehensive medication management services with patients and providers to ensure optimal patient care and education in regards to medications, ensuring medications are indicated, effective, safe, and convenient.
  • Participates in pre-visit planning and coordinates care plan with the patient's care team and caregivers.
  • Documents care in electronic health record, consistent with clinic policies and expectations
  • Participate in the development of organization wide guidelines and policies related to medication management, including clinical practice guidelines, pharmacy interchange policies, etc.
  • Provide recommendations to improve the delivery of clinical services and medication management in accordance with internal policies and external regulatory requirements.
  • Provides comprehensive, general and patient-specific drug information to providers, nurses, and other staff members.
  • Provides education and training regarding common diseases, medication use and monitoring for acute and chronic diseases, and prevention.
  • Develops and maintains clinical pharmacy programs using population health methodology
  • Perform other duties as assigned.

SUPERVISORY RESPONSIBILITIES:

This job has no supervisory responsibilities.

QUALIFICATIONS:

Education and/or experience:

  • BA/BS degree plus at least eight years of experience, or an advanced degree plus at least six years of experience, or a terminal degree plus at least two years of experience to include a Bachelor's degree or Doctor of Pharmacy degree from an accredited school of pharmacy
  • Completion of residency with ambulatory care focus (PGY1 or PGY2), or at least 2 years experience as a practicing pharmacist in an outpatient clinical setting
  • Minnesota pharmacist license in good standing

Certificates, licenses and registrations required: PharmD; active, unrestricted license to practice in Minnesota; BLS CPR, First Aid or other needed certifications when requested; proof of current COVID-19 vaccination.

Computer skills required: Internet Software; Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook); Presentation software (PowerPoint); experience using EMR preferred.

Other skills required: Works well independently and on a team; excellent communication, customer service, and case management skills.
