Job Summary: The Care Navigator will provide Chronic Care Management and Care Coordination to patients with multiple chronic conditions. Functioning as an integral part of the care team, this role provides ongoing activities and follow-up to the patient population that occurs beyond the walls of the health center and provider visits. The Care Navigator will perform appropriate assessments, assist in goal setting and self-management, and follow care plans over the telephone. This position's homesite is at the Denton Health Center providing support to Federalsburg Health Center.

Education and Experience:

  • High School diploma or equivalent
  • Certificate of completion from an accredited nursing assistant or medical assistant program; certificate must remain current
  • Minimum 2 years medical office experience preferred
  • Minimum 2 years EMR system experience preferred Strong communication skills
  • Strong organizational and time management skills
  • Basic understanding of medical terminology
  • Ability to understand and carry out written, oral and/or graphic instructions
  • Ability to interact with patients, medical and administrative staff, and the public effectively
  • Intermediate proficiency with computers and telephone use
  • Positive and proactive attitude; team player
  • Goal orientated
  • Must be punctual and have reliable transportation

Duties/Responsibilities:

  • Enroll patient in the specific programs via telephone by conducting initial outreach calls, assessments, and development of patient-centered care plans.
  • Provide timely telephonic follow-up by keeping track of scheduled tasks.
  • Adjust care plans as needed based on patient changing conditions and provider’s recommendations.
  • Maintain on-going communication with Care Coordinator Nurse, provider, and clinical staff electronically and by sharing patient care plans and summaries.
  • Provide patient services, including:
  • Maintain and monitor assigned patient roster.
  • Conduct patient outreach and telephonic care coordination with chronically ill patients.
  • Provide motivational support to maximize adherence to care plan.
  • Effectively utilize Electronic Health Record systems to gather patient medical history.
  • Track time spent for Chronic Care Management cases.
  • Effectively identify any patient issues and escalate them as appropriate.
  • Use appropriate community resources to address patients’ needs.
  • Completes all necessary documentation in the patient’s medical record required for Chronic Care Management.
  • Performs Other Duties as Required
  • Consistently displays professionalism during interaction with CCHS customers, staff and clinicians.
  • Responsible for conducting themselves in an ethical manner and reporting possible violations through the appropriate channels. Employees must be careful in both words and conduct to avoid placing or appearing to place pressure on subordinates or coworkers that could cause them to violate these standards of conduct or to deviate from accepted norms of ethical business practice.
  • Required to comply with the Health Insurance Portability and Accountability Act (HIPAA) regulations.
  • Regular, reliable attendance is a requirement of this job.

Schedule: 8 hour shift - Monday to Friday
