YoloCares (formerly Yolo Hospice) is Joint Commission accredited and the only organization in the Sacramento region to be accepted into the esteemed National Partnership for Hospice Innovation.
The Home Health Aide will provide personal care and assistance to hospice patients in a facility as part of the Yolo Hospice Interdisciplinary Team. In this role, your will follow universal precautions in delivering patient care, perform routine procedures, and observe and monitor the patients’ needs as outlined in the Plan of Care.
Requirements:Job Qualifications:
- Valid and active Certified Nursing Assistant (CNA) license and Home Health Aide (HHA) certification is required.
- Must be CPR Certified.
- Minimum 1-year Hospice experience preferred.
- Ability to make clinical observations and clearly communicate verbally and in writing.
- Strong interpersonal skills and ability to quickly build rapport with others.
- Ability to successfully meet agency background screening requirements.'
- Ability to safely lift at least 50lbs

Pay range: $25-$27/hour
