The Peritoneal Nurse will train and follow up on all patients selected for Peritoneal Dialysis. This position performs duties as assigned by the Facility Administrator and on occasion the Charge Nurse.


Duties:
1. Enforce all company, facility, and CMS approved procedures and policies
2. Train patients (and/or family) who have chosen Peritoneal Dialysis in the practice of performing Peritoneal Dialysis within their home setting. Ensuring competence of trainee before allowing them to perform the job independently
3. Periodically re-assessing patient’s skills and competence and re-training if needed
4. Maintain a schedule to ensure that the patient receives a yearly physical examination
5. Make contact with the peritoneal dialysis patient at least monthly by phone or during clinic visits
6. Maintain a schedule that ensures each patient received a clinic visit monthly
7. Communicate with appropriate interdisciplinary team members on a regular basis
8. Obtain routine laboratory work to include routine adequacy testing
9. Provide the patient with instruction on how to contact staff at any time in the event of an emergent need
10. Make surveillance of the patient’s home adaption, including provisions for visits to the home
11. Ensure the patient is provided consultation with a Registered Nurse, Social Worker and Dietitian
12. Establish and maintain a patient chart which ensures continuity of care. Accurate collection of patient treatment home data on a monthly basis and address issues of non-adherence during clinic visits and CIPA
13. Ensure through a contract agreement, proper installation and maintenance of equipment when required
14. Establish and maintain an ordering system that ensures the patient received all supplies in a timely manner
15. Communicate in writing at least monthly through progress notes that document continuity of care
16. Obtain data and participate in the QAPI meeting monthly
17. Implement and maintain CIPA’s and POC’s on all new admissions and ongoing for established patients
18. Review of Peritoneal supply cost monthly
19. Responsible for promoting the program and modality by coordinating with outpatient CKD clinics and in-center clinics through education of treatment options
20. Demonstrates effective use of supplies and labor hours
21. May cross-train as in-center Hemo nurse to meet the facilities need which include and may not be limited to the following:
a. Perform duties as assigned to meet the patient care and/or operational needs of the facility
b. Prepare and administer medications as ordered by the physician
c. Provide patient education and follow up as needed
d. Complete pre and post treatment physical assessments
e. Initiate and terminate patient treatments to include connecting and disconnecting catheters/access sites and cleaning catheter/access sites per protocol
f. May reinforce patient education in-center setting as needed under the direction of the Facility Administrator, Charge Nurse or Registered Nurse
g. Assist with programs target goals for patient outcomes in accordance with quality patient care and company goals
h. Knowledge of and comply with appropriate healthcare professionals practice act requirements
i. Use appropriate safety measures including personal protective equipment (PPE) as necessary
j. Obtain pre and post treatment weight of patient per protocol
k. Safely initiate and terminate dialysis treatments per facility protocol, and within scope of practice as allowed by state regulations
l. Responsible for accurate documentation of information related to patient treatment
m. Report machine problems to the Biomedical Technician and Facility Administrator
n. Take vital signs per protocol during patient treatment and document on treatment record as required.
22. Remove disposables and clean station after each training
23. Be familiar with emergency equipment and all emergency operational measures
24. Participate in staff meetings as required. Attend in-services and continuing education sessions in compliance with company policy and procedure
25. Participate in team concepts and promote a team effort, perform duties in accordance with company policies and procedures
26. Other duties as assigned


Requirements:

1. High school graduate or equivalent
2. CPR certification required. Certification must be maintained as current and in good standing
3. Current RN license in relevant state. License must be maintained as current and in good standing
4. 12 months clinical experience required
5. 6 months experience in Peritoneal Dialysis preferred
6. Confirmation of ability to differentiate primary colors
7. Demonstrated working knowledge of the English language and ability to communicate in writing as well as verbally
8. Must have basic computer skills; proficiency in all NRC clinical applications required within 90 days of hire
9. Must be able to organize time and tasks well.
10. Must meet any practice requirement(s) for the relevant state
11. Ability to meet minimum hospital/agency hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment
