Two Years of Home Health (Personal, Skilled, Family Care or Hospice) Required.

We are seeking a part-time Home Health Services Director (HHSD) to join our team and lead the CHAP & Medicare credentialing process. This role offers flexible hours and involves supervising 1-2 PRN nurses, assisting with skilled and Medicare surveys, and performing PRN fieldwork. Leadership meetings are primarily held via Zoom, with occasional time required in the Mesa office for supply pickups, interviews, surveys, meetings and procedure binder updates. The HHSD will need to research, and implement CHAP, Arizona skilled and Medicare processes and standards.


    •    RN skilled field care assignments as needed & fitting your schedule, such as wound care, IVs, Medication Management, Post Surgical care. (PRN work will be compensated at a higher rate including travel)
    •    HHSD oversees 1-2 field nurses providing home care.
    •    Any experience with Medicare surveys and procedures is a plus.
    •    Our office is in Mesa, however most leadership activity is via Zoom.
    •    We are working through various credentialing like CHAP and expect hours to increase in Q4. However, you will still have the flexibility to work the hours you prefer.

As a HHSD/RN, here’s a big-picture view of what you’ll do:

  • Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards
  • Coordinate and manage a team of nurses, therapists, CNAs, aides and social workers to ensure client and patient goals are met
  • Recruit, hire, orient, train, supervise and retain qualified clinicians and caregivers
  • Ensure medical supplies/equipment are delivered and staff are trained on care procedures
  • Assess patient/client needs, resolve issues, and ensure a high level of satisfaction
  • Effectively manage financial resources related to the delivery of quality patient/client care

A few must-haves for a Director of Healthcare Services/RN:

  • Graduate of an accredited nursing program and active RN or BSN license in Arizona
  • Minimum of 2 years of nursing experience in home healthcare
  • CPR certification 
  • Extensive knowledge of state and federal home health laws and regulatory standards
  • Excellent management, organizational, problem-solving, communication and interpersonal skills
