Home Health Physical Therapist Assistant PTA

Pathwell Health is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality home health care services. As a PTA, you will work under the supervision of a licensed Physical Therapist to deliver ongoing therapy services to patients in their homes.
Key Responsibilities- Provide therapeutic exercises as planned, delegated, and supervised by the Physical Therapist.
- Effectively communicate with the patient's care team and follow instructions given by the assigned Physical Therapist.
- Perform ongoing therapy services, including application of heat and cold, massage, gait training, and other techniques as needed.
- Accurately document care and patient progress in a timely manner, meeting established deadlines and regulatory requirements.
- Confer with the licensed PT at least every 30 days to review the patient care plan and make any necessary modifications.
- Attend meetings and in-services as required.
- Graduate of a Physical Therapy Assistant Program, accredited by the American Physical Therapy Association or equivalent program.
- Current and valid California Physical Therapist Assistant license.
- Recent work experience in home health is preferred.
- Driver's License, Car Insurance, and Car Registration.
- Reliable transportation and ability to drive in all types of weather conditions.
- Ability and willingness to follow agency policies and procedures and work as a team member.
- Strong interpersonal skills and good judgment.
