The Community Support Specialist provides community support and case coordination services for a caseload of individuals with disabilities/intellectual disabilities or individuals with mental illness. The
Community Support Specialists works with the people we serve using a true person center approached in order to foster relationships between individuals and their communities. To learn more about this position, please read the summarized list of duties below.
- Assesses consumer's overall functioning, identifying problems, needs, skill strengths and deficits, and assumes a lead role in the consumer centered planning process developing implementation strategies.
- Implements selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports.
- Ensures linkages to community service providers, entitlement programs and housing alternatives.
- Advocates for individual's basic need, development of natural resources, and personal independence through teaching self-advocacy skills.
Our minimum requirements include either being a Qualified Intellectual Disability Professional (QIDP) or qualify for QIDP with a Bachelor's Degree in the field of Human Services and at least one year of direct care experience working with development/intellectual disabilities. Must have reliable transportation, valid driver's license, and meet insurability requirements.
Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to work with persons with developmental disabilities. Must have Mental Health Professional (MHP) status to work with persons with mental illness. Employment with Bridgeway is conditional upon completion of all governing required background checks.
