Broad Statement of Responsibilities: Provides clinical services, clinical supervision, case conceptualization, client assignment at APN Lodge. The individual in this role is responsible for the overall quality of care, outcomes management, staffing and service levels. Ensures appropriate and effective staffing levels and resource management. Participates and collaborates with other disciplines at APN to enhance quality and service level outcomes and assists in program development. This individual provides individual and group counseling services to patients and families while leading an Interdisciplinary team which develops individualized treatment plans for each patient. This position will also be responsible for Evening Intensive Outpatient Program (EIOP) for SUD/Co-Occurring Groups.


Position Duties and Responsibilities –?Responsibilities include but are not limited to:

Oversee Clinical delivery for the assigned treatment, including RTC/ PHP/TIOP/TOP Programs

Oversee and manage Client Census for the team

Conduct and supervise Clinical Assessments for admission

Collaborate with Referral Sources

Referral Responsibility

Admission contact w/referral sources

Bi-weekly updates throughout the entire process

Provide documentation/ Complete Assessments as needed

Clinical Conceptualization/ Summary to support T&P Process

Goals

Assist with Program Development / Curriculum for afternoon offerings

Responsible for scheduling and management of treatment at the direction of clinical leadership

Continue to bridge and develop relationships with Referral Sources and Multidisciplinary team

1. SUPERVISION

Provide effective role modeling and leadership to the treatment team, including supervision for therapists\addiction\mental health counselors

Assist in completing 90-day and annual performance evaluations per established timelines when indicated.

Ensure coverage for all scheduled services.

2. BIO-PSYCHO-SOCIAL ASSESSMENTS

Ensure collection of thorough, relevant and pertinent data to inform treatment recommendations and planning.

Ensure production Bio-Psycho-social assessments in a timely manner.

Follow TJC, CARF and All Points North Lodge CQI processes.

Assess consumers for appropriate admissions and therapist assignments.

3. DIAGNOSTIC SUMMARIES.

Collect pertinent data.

Produce diagnostic summaries in a timely manner.

Follow TJC, CARF and All Points North Lodge CQI standards.

4. (Person Centered) TREATMENT PLANS

Ensure the team devises treatment plans that reflect current evidence-based practices

Ensure completion of treatment plans in a timely manner.

Follow TJC, CARF, OBH and All Points North Lodge CQI standards.

5. DISCHARGE PLANNING

Formulate appropriate post-discharge placement plans that include services for consumer and family that maximize potential for success.

Inform administrative staff of pending/planned discharges.

At discharge, assess consumer’s appropriateness for discharge and continued care needs.

6. FAMILY THERAPY

Provide, either directly or through program staff, individual family therapy sessions for each family according to person centered plan.

Maintain confidentiality.

Use and accept co-counselor(s) for observation/supervision and training.

7. INDIVIDUAL THERAPY

Provide therapy to clients as is deemed needed by treatment plans.

Use and accept co-counselor(s) for observation/supervision and training.

8. GROUP THERAPY

Provide therapy to clients on a group basis as is deemed necessary by treatment plans.

Organize and conduct weekly specialty groups as assigned.

Use and accept co-counselor(s) for observation/supervision and training.

9. TREATMENT TEAM

Facilitate regularly scheduled treatment team meetings in conjunction with those run by the Manager of Clinical Operations.

Provide clinical expertise and interact with multi-disciplinary treatment team.

Document meetings and recommendations within the client medical record.

10. TREATMENT PLAN REVIEWS

Review treatment plans on a scheduled basis with treatment team following TJC, CARF, OBH & APN standards.

Provide written treatment plan reviews for placement in clinical records as assigned.

Obtain client signatures on all necessary forms.

11. CONTACT WITH REFERRAL SOURCES

Maintain regular contact with referral sources as indicated.

Update on client’s progress.

Develop appropriate discharge plans collaborating with referral sources.

Prepare and conduct concurrent reviews to referral sources.

12. DOCUMENTATION

Responsible for timely completion of medical record documentation.

Documentation of all outside contacts as well as therapy sessions will be maintained.

13. CQI & TEAMS

Attend and participate in one or more CQI teams annually.

Participate in peer record review process.

OBH data entry where indicated.

14. STAFF RELATIONSHIPS

Maintain productive, effective and professional relationships with all disciplines.

Keeps supervisor informed.

Open to feedback and supervision.

Additional training as assigned by management 16. Evening Intensive Outpatient Program (EIOP) for SUD/Co-Occurring Groups

  • Provide complementary in-person IOP program for APN clients and outside referrals as part of continuum of care offerings
  • Provide groups and individual services to fulfill IOP requirements for 8 weeks programming
  • Administration and management of clients within and outside of APN services
  • Develop curriculum, SOPs, financial interface, marketing material, workflow, documentation requirements, and executable program guidelines
  • Interdepartmental communication with APN staff to facilitate integration
  • Maintain compliance within the scope of care delivered to maintain APN accreditation and organizational objectives
  • Provide weekly data to leadership for management of service line
Requirements:

Qualifications: The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Education: This position requires CO licensure, a Graduate Degree in Social Work, Counseling, or Psychology from an accredited college or university. Ideally, will possess Dual Licensure: Colorado OBH credentialing (CACIII)\ licensing (LAC) in addiction counseling and OBH mental health professional licensure (or equivalent and ability to acquire equitable Colorado credentials).

Experience: Five years clinical and three years supervisory experience working with clients in SA\MH settings. Must demonstrate working knowledge of clinical supervision with behavioral healthcare professionals, as well as a documented history of facilitating clinical supervision for candidates seeking or maintaining professional credentials. Experience also to include: family, group, and individual therapy/counseling

Special Skills & Knowledge: Strong verbal and written skills are required
