This is responsible, professional work assisting eligible individuals aged 5- 20 (under 21) who experience SMI/SED in gaining access to needed medical, social, educational, and other services. The Intensive Care Coordinator facilitates a collaborative relationship among a youth with SED, their family, and involved child-serving systems to allow and support the parent/caregiver in meeting their youth's needs.

The High-Intensity Care Coordinator (HICC) utilizes the Wraparound Informed Approach to facilitate the development and ongoing collaboration of Child & Family Care Team to provide a single point of accountability for ensuring that medically necessary services and supposed are accessed, coordinated, and delivered in a strength-based, individualized, family/youth driven, and ethnically, culturally, and linguistically relevant manner.

DESCRIPTION OF DUTIES:

Provide the essential services of the HICC, which include the Needs Assessment, Care Planning, Service Arrangement, Social Support, Reassessment and Follow-up, and Monitoring.

Provide a comprehensive home-based assessment (CANS) within fourteen (14) days of acceptance of HICC services and periodic reassessment (minimally every six months) of individual needs, to determine the need for any medical, educational, social, or other services.

Facilitate and convene the development of the Child & Family Care Team within 30 days of acceptance of HICC services and minimally every 6 months thereafter.

Within 30 days of initiation of HICC services and in collaboration with the Child & Family Care Team, develop a systematic, individual-focused, and strength-based HICC Plan of Care that lists actions required to meet each of the identified needs of the recipient.

Develop a Crisis Stabilization & Support Plan intended to provide a means of proactively planning for ensuring safety, de-escalation, and minimizing risk of out-of-home placement in moments of crisis.

Provide Crisis Intervention as needed to de-escalate potential crisis events in effort to support the recipient in remaining in-the-home, and as needed, linkage to external supports.

Provide contacts with recipient and/or collateral on a minimum of 1-3 contacts per week.

Provide referral and related activities such as working directly with the recipient, scheduling appointments for the recipient, and helping to obtain needed services.

Assist the recipient in expanding or establishing their personal support system through social support, advocacy, and linking the recipient with appropriate persons, support groups, or agencies.

Evaluate the recipient's progress towards accomplishing the goals listed in the HICC Care Plan at intervals of 6 months or less.

Determine needs of SED children by using the CANS Assessment.

Assist the individual for such other services as may be appropriate.

Transport clients to services, as well as conduct outreach visits.

Participate in training activities as required by Carastarto increase and enhance professional and therapeutic skills.

Complete reports required for client records as well as all forms and paperwork required by Carastar.

Perform other related duties as assigned by supervisor.

REQUIREMENTS:

Proficient computer skills to include the use of Windows operating system, typing, and proficient use of Excel.

Proficient knowledge on the use Electronic Health Record and ability to provide timely documentation of services provided.

Knowledge of the special needs and behavioral characteristics of seriously emotionally disturbed children and adolescents.

Knowledge of interviewing techniques and principles.

Skills in interpersonal relationships and group dynamics.

Knowledge of the use and side effects of psychotropic medication.

Knowledge of the legal and ethical issues relative to confidentiality of client records.

Knowledge of casework principles and methods related to counseling.

Ability and willingness to document activities and maintain records.

Ability and willingness to provide mental health services in non-traditional settings.

QUALIFICATIONS:

Bachelor's degree in behavioral science or related field from a recognized college or university plus at least one year of related experience.

Must meet requirements for approved Medicaid Provider status.

Must hold a valid driver's license and maintain a driving record that is acceptable to the Carastar Health's insurance carrier.

Must maintain at least liability coverage on personal vehicles.

Within three months of hire, must satisfactory completeADMH approved Intensive Care Coordination Curriculum which is based upon Systems of Care principles and the Wraparound planning process
