This is responsible. professional work assisting eligible individuals aged 5- 20 (under 21) who experience Serious Mental Illness/Serious Emotional Disturbance (SMI/SED) in gaining access to needed medical, social, educational, and other services. The Intensive Care Coordinator facilitates a collaborative relationship amongyouth, their family, and involved child-serving systems to allow and support the parent/caregiver in meeting their youth's needs.

The Care Coordinator provides these services through telephone or face-to-face contact with recipients. This is accomplished through the case management services of needs assessment, case planning, service arrangement, social supports, re-assessment, follow-up, and monitoring. The care coordinator will cover a specific county or counties within the catchment area.Specific assignments will be discussed if an offer is extended.

DESCRIPTION OF DUTIES:

Provide initial contact with recipient and family within 24 hours of receiving a referral for services. Offering Low Intensity Care Coordination (LICC) service and scheduling an intake interview within seven days.

Provide a comprehensive home-based assessment (CANS) within 14 days of acceptance of LICC services and periodic reassessment (minimally every 6 months) of individual needs, to determine the need for any medical, educational, social, or other services.

Develop Crisis Stabilization & Support Plan intended to provide a means of proactively planning for ensuring safety, de-escalation, and minimizing risk of out-of-home placement in moments of crisis.

Provide Crisis Intervention as needed to de-escalate potential crisis events in effort to support the recipient in remaining in-the-home, and as needed, linkage to external supports.

Provide contacts with recipient a minimum of 1 contact per week during the first 4 weeks of service, and a minimum of one contact per month thereafter with more as needed.

Provide referral and related activities such as working directly with the recipient, scheduling appointments for the recipient, and helping to obtain needed services.

Assist the recipient in expanding or establishing their personal support system through social support, advocacy, and linking the recipient with appropriate persons, support groups, or agencies.

Evaluate the recipient's progress towards accomplishing the goals listed in the LICC Care Plan at intervals of six months or less.

Assist the individual for such other services as may be appropriate.

Transport clients to services, as well as conduct outreach visits.

Participate in training activities as required by Carastar Health to increase and enhance professional and therapeutic skills.

Complete reports required for client records as well as all forms and paperwork required by Carastar,including Daily Service Reports and other documents as needed.

Perform other related duties as assigned by supervisor.

REQUIREMENTS:

Basic computer skills to include the use of Windows operating system, typing, and basic use of Excel.

Basic knowledge on the use Electronic Health Record and ability to provide timely documentation of services provided.

Knowledge of the special needs and behavioral characteristics of seriously emotionally disturbed children.

Knowledge of the use and side effects of psychotropic medication.

Ability to interact and communicate effectively with other professionals and other agencies.

Ability to organize and manage one's own activities with minimum of direct supervision.

Ability to quickly establish rapports with recipients and families.

Knowledge of the legal and ethical issues relative to confidentiality of client records.

Knowledge of casework principles and methods related to counseling.

Ability and willingness to document activities and maintain records.

Ability and willingness to provide mental health services in non-traditional settings.

Ability to operate a motor vehicle.

QUALIFICATIONS:

Bachelor's degree in behavioral science or related field from a recognized college or university. Some experience preferred.

Must meet requirements for approved Medicaid Provider status.

Must hold a valid driver's license and maintain a driving record that is acceptable to Carastar Health's insurance carrier.

Must maintain at least liability coverage on personal vehicles.

Within three months of hire, must satisfactory complete an ADMH approved Intensive Care Coordination Curriculum which is based upon Systems of Care principles and the Wraparound planning process
