Key Responsibilities:
  • Provide therapeutic and case management services to adults with substance use disorders and psychiatric disabilities.
  • Help individuals enhance their functioning within the community by implementing care plans developed in collaboration with the primary therapist.
  • Ensure comprehensive and coordinated care for each client, including assisting with applications for benefits and health insurance (SSI, SSDI, Medicaid, Medicare).
  • Facilitate access to essential resources such as psychiatric care, housing, rehabilitation programs, and socialization activities.
  • Conduct outreach to clients on the caseload, maintaining their engagement in services.
  • Maintain regular communication with other treatment providers to ensure holistic care.
  • Complete required documentation in the Electronic Health Record (EHR) system in a timely manner.
  • Meet established billing quotas on a weekly, monthly, and quarterly basis.
Qualifications:
  • Associate degree in psychology, social work, or a related field; Master’s degree preferred.
  • CDCA and a valid Ohio Driver’s License.
  • Minimum of two years of experience working with adults with substance use disorders and/or mental health diagnoses.
  • Knowledge of trauma and its effects.
  • Strong verbal and written communication skills.
  • Excellent organizational skills and the ability to manage multiple tasks.
  • Strong interpersonal skills, with the ability to effectively engage with clients, team members, and the public.
  • Flexibility to adapt to clinical needs and emergency situations.
Physical Requirements:
  • Regular sitting, use of hands and fingers, talking, and hearing.
  • Frequent walking, balancing, stooping, kneeling, and/or crouching.
  • Occasionally lifting/moving up to 15 pounds.
  • Vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Keyboard data entry is required.
